摘  要:目的分析甲状腺微小癌(TMC)的超声图像特征,以期提高超声对其诊断的正确率。方法对经手术病理确诊的83例TMC(101个病灶)的超声图像作回顾性分析,分析其超声图像、血流分布特征及颈部淋巴结受累及情况。结果83例TMC(其中PYMC82例,滤泡状TMO1例,共101个病灶)术前超声发现病灶69倒(83个),超声表现为不均低回声81个(97.6%),不规则形72个(86.7%),边缘不整齐72个(86.7%),85个TMC结节A/T平均值为091(0.50~1.50),69个(83.1%)I-MC结节A/T≥1,83个TMC结节均无声晕,病灶内见微钙化42个(50.6%);彩色多普勒血流显像:I型25个(30.1%)、II型42个(506%)、III型16个(193%);颈部淋巴结转移22例(26.5%)。结论PYMC的超声图像特征为病灶多呈不均低回声,形态不规则、边界模糊、不整齐,A/T多≥1,结节内部多见点状或簇状钙化;对于PTMC的超声诊断要依靠以上多个超声图像特征性指标综合判断。Objective To aim to elevate diagnostic correct rate by analyzing the ultrasonographic features of thyroid microcarcinoma(TMC). Methods We analyzed 83 cases(101 nodu|es)of TMC which were 1.0 cm or less in diameter histopathologically.All patients accepted two- dimensional and color Doppler ultrasound. To analyze their ultrasonographic features,bloodstream distribution features and metastatic in lymph node of neck. Results 83 cases(pillary thyroid microcarcinoma 82 cases, follicle thyroid microcarcinoma one case,101 nodules in total )of TMC were found 69 cases(83 nodules) on ultrasonography, These characteristics of 83 nodules were hypoecho and heterogeneous echo solid nodule (97.6%),irregular shape(86.7%),ill defined edge (86.7%), mean value of anteroposterior and transverse diameter ratio(A/T): 0.91(range from 0.50 to 1.50), 69 nodules (83.1%) A/T≥ 1,absent halo sign in 83 nodules which were found on ultrasonography, microcalcifications(50.6%),color doppler flow imaging:type I 25 nodules (30. 1%),type II 42 nodules (marginal blood flow pattem,50.6%),typem 16 nodules (19.3%);Metastatic in lymph nodes of neck 22 nodules (26.5%). Conclusion Their characteristics of PTMC were more often hypoecho and heterogeneous echo solid nodule,irregular shapes,ill defined edge,A/T≥ 1,microcalcifications;Diagnosis of PTMC relied on synthetic judgement more features on Ultrasonography.
