摘 要:目的对比感染后功能性消化不良(functional dyspepsia,FD)、非感染后FD及健康志愿者十二指肠粘膜肥大细胞,嗜酸性粒细胞数量的变化,并探讨胃肠道感染对FD发病的影响。方法选取2011年10月—2012年10月就诊的患者作为实验对象,既往6个月内具有胃肠道感染史的患者共28例作为感染后FD组,,既往6个月内无胃肠道感染史的患者共39例作为非感染后FD组,对照组选用健康志愿者21例。十二指肠粘膜组织用10%甲醛固定,梯度酒精脱水,常规石蜡切片厚6μm。甲苯胺蓝染色法染色后光镜下观察肥大细胞;HE染色法染色后光镜下观察嗜酸性粒细胞。每张染色切片随机选5个高倍镜视野(×400倍),计数染色细胞数量,细胞计数以5个视野中细胞数之和表示。计量资料采用t检验,P<0.05为差异有统计学意义。结果三组肥大细胞计数感染后FD组十二指肠球部肥大细胞(duodenal bulb mast cell,D1MC)、十二指肠降部肥大细胞(descendant duodenum mast cell,D2MC)分别为(124.91±14.31)、(125.28±12.40)个,非感染后FD组分别为(120.26±13.75)、(121.32±13.53)个,对照组分别为(105.29±20.81)、(107.62±17.64)个,三组比较差异均有统计学意义(均P<0.05)。三组嗜酸性粒细胞计数感染后FD组十二指肠球部嗜酸粒细胞(duodenal bulb eosinophils,D1E)、十二指肠降部嗜酸粒细胞(descendant duodenum eosinophils,D2E)分别为(21.62±6.26)、(25.23±5.18)个,非感染后FD组分别为(21.46±5.03)、(23.65±5.71)个,对照组分别为(21.25±5.78)、(19.38±5.06)个,三组D2E比较差异均有统计学意义(均P<0.05)。结论 FD患者十二指肠粘膜中嗜酸性粒细胞、肥大细胞数量增加。十二指肠存在免疫激活现象,感染可能是胃肠道免疫激活的始动因素之一。Objective To compare the number of duodenal mucosal mast cells (MC)and eosinophils (E)in patients with post -infectious functional dyspepsia( PI -FD), and healthy volunteers, and to discuss the influence of gastrointestinal in- fection on FD. Methods 67 treated FD cases were recruited in the study,of whom, 28 cases who had a positive history of gastroenteritis in the past six months were considered as PI -FD group, and 39 cases who had not as FD group. 21 healthy volunteers were selected as control group, Duodenal mueosa was fixed with i0% formalin, dehydrated with graded alcohol, and cut into paraffin sections, 6 p.m in thickness (toluidine blue, hematoxylin, eosin dye from Wuhan Boster). Toluidine blue staining was used for observing mast cells under light microscope, and HE staining for eosinophils. Five high -power field ( x 400) were randomly selected for observing each stained section, the number of stained cells was counted, which was represented by the total number of cell count in five visions. T - test was used for measurement data, the result of P 〈 0.05 was considered to be statistically significant. Results The number of duodenal bulb mast cells and descendant duode- num mast cells was ( 124.91 ~ 14.31 ) and( 125.28 ~ 12.40) respectively in PI - FD group, ( 120.26 ~ 13.75 ) and( 121.32 ~ 13.53 ) respectively in FD group, and ( 105.29 ~ 20.81 ) and ( 107.62 ~ 17.64) respectively in control group. There was significant difference between three groups ( P 〈 0.05 ). The number of duodenal bulb eosinophils and descendant duode- num eosinophils was (21.62 +6.26) and (25.23 ~5.18) respectively in PI -FD group, (21.46 ~5.03) and (23.65 ~ 5.71 ) respectively in FD group, ( 21.25 ~ 5.78) and ( 19.38 ~ 5.06) respectively in control group.
