摘  要:目的:探讨三维CT测量及移植骨成形技术在下颌骨节段性缺损修复重建应用的方法及效果.方法:在CT三维重建图像上,对12例不同原因造成的下颌骨节段性缺损患者完成缺损测量,获得下颌支的高度和下颌体及颏部的长度,以及下颌支与下颌体、下颌体与颏部的角度,根据测量数据对血管化游离腓骨瓣或髂骨瓣塑形植入.结果:术后所有患者伤口均Ⅰ期愈合,随访0.5~4.0年,颜面外形恢复良好,重建下颌骨弧度恢复满意,植入骨瓣截骨各段及其与下颌骨残端间均骨性愈合,无供区与受区并发症.结论:利用CT三维重建技术修复重建下颌骨可提高手术的精确性,简化移植骨的塑形操作.Objective:To observe the effects of three-dimensional CT measurement and bone graft shaping technique in segmental mandibular defect restoration.Methods:From July 2008 to January 2012,12 patients with mandibular segmental defect were included.The height of mandible ramus,the length of mandible body and chin,the angle between mandible ramus and body,and the angle between mandible body and chin were measured on three-dimensional CT images.According to the measurement data,vascularized fibula flap or iliac flap were shaped and transplanted.Results:After operation wounds in all the patients were healed in stage Ⅰ.At the end of the postoperative follow-up (half year to 4 years),good facial appearance,satisfactory mandible contour and perfect bone union between the implanted bone and residual mandible bone were achieved without donor or recipient site complication.Conclusion:Three-dimensional CT measurement can improve the surgical accuracy and simplify the shaping procedure of bone graft.The technique can be widely used in restoration of segmental mandibular defect.
