Superbond粘结系统在前牙重度牙周炎中的临床应用

摘　　要：目的观察Superbond粘结系统在重度牙周炎中作为牙周夹板固定松动前牙的疗效。方法选择南京大学医学院附属口腔医院牙周科就诊的重度牙周炎导致前牙松动的患者64例,随机分为两组,实验组(Superbond固定组)和对照组(钢丝结扎+树脂组),各32例。进行全口牙周基础治疗,口腔卫生指导。观察1个月,口腔卫生保持良好,牙周炎症得到控制,消除咬合创伤后固定。每3个月复查维护一次,连续观察12个月。疗效评价为无效和有效。通过测定平均灰度值比较固定前后数字化曲面断层片上的牙槽骨密度的变化。采用问卷调查的方式对两种治疗方法的美观性和舒适性进行评价。结果实验组29例有效,对照组28例有效。实验组固定12个月后牙周探诊深度(PD)较固定前减小,牙槽骨密度较固定前增加,具有统计学差异(P<0.05)。患者对实验组美观性和舒适性调查满意度高于对照组。结论在选择合适病例的前提下,Superbond粘结系统作为牙周夹板固定松动前牙可取得提高咀嚼力,改善生活质量的目的。Objective To observe and evaluate the curative effects of Superbond adhesive system as the periodontal splint on loose an-terior teeth of patients in treating advanced periodontitis. Methods 64 patients with advanced periodontitis of anterior teeth were selected and randomly divided into two groups,experimental group （superbond fixation group） and control group （wire ligation and resin fixation group） ,with 32 patients for each group. Full-mouth periodontal treatment and oral hygiene instruction were performed to maintain good o-ral hygiene,control periodontal disease and eliminate occlusal trauma. After one month,two fixation methods were used in experimental group or control group respectively. The review and maintenance were suggested every three months, and continuous observation was done for 12 months. The treatment response was evaluated as effective and ineffective. The change of alveolar bone density was compared before and after treatment according to the gray scale on panorama radiography. A questionnaire survey was conducted to evaluate the aesthetics and the degree of comfort of two treatment methods. Results In experimental group,29 cases were evaluated as effective while 4 cases were considered as ineffective in control group. Compared with that before treatment,periodontal depth （PD） decreased and the alveolar bone density increased after 12 months in experimental group,and there was statistically significant difference （P 〈0.05）. According to the survey,aesthetics and the degree of comfort was higher in experimental group. Conclusions For appropriate cases,Superbond bonding system can be used as periodontal splint to fix loose teeth in order to improve chewing ability and the quality of life.
