血浆白细胞介素4对脑胶质瘤患者预后的意义

摘　　要：目的流行病学调查表明脑胶质瘤患者过敏反应的发生率降低，我们在脑胶质瘤患者血浆中检测了参与过敏反应的重要细胞因子白细胞介素-4，并探讨了血浆白细胞介素-4水平的临床意义。方法收集并检测了217例脑胶质瘤患者术前血浆样本中自细胞介素-4浓度，并评估了血浆白细胞介素-4水平与肿瘤级别和患者预后的关系。结果血浆白细胞介素-4水平在不同级别的脑胶质瘤患者间无明显差异。在Ⅳ级胶质瘤患者中血浆白细胞介素-4与患者生存期相关（HR0．296，95％C10．134～0．653，P=0．003）。血浆白细胞介素-4高的患者生存期长于血浆白细胞介素-4低的患者但未达统计学意义（高白细胞介素-4和低白细胞介素-4生存期分别为31．92个月、20．55个月，P=0．079）。在Ⅲ级胶质瘤患者中血浆自细胞介素-4与预后无明显相关。结论血浆白细胞介素-4与Ⅳ级胶质瘤患者预后相关，需要进一步扩大样本验证及研究其潜在的生物学机制及治疗价值。Objective Epidemiology research has found less reported allergic condition in patients with gliomas. We tested plasma interleukin -4, which plays important role in allergic reaction, in patient with gliomas and determined its clinical significance. Methods A total of 217 patients with gliomas were studied. Plasma interleukin-4 concentration was determined and correlated with tumor grade and patients＇ survival. Results Plasma interleukin -4 did not have significant difference between gliomas with different pathological grades. In grade IV gliomas, plasma interleukin- 4 was positively correlated with patients＇ survival time （ HR 0. 296, 95% CI 0. 134 - 0. 653, P = 0. 003 ）. The patients with higher than median plasma interleukin -4 had longer survival time than the patients with low plasma interlenkin -4 （high interleukin-4 vs. low interleukin-4, 31.92 months vs. 20. 55 months, P =0. 079）. In grade Ⅲ glioma patients, this correlation was not significant. Conclusions Plasma interleukin -4 was correlated with patients＇ prognosis in grade Ⅳ gliomas. Further studies with expanded population and investigation on the underlying biological mechanisms are warranted.
