显微喉镜下低温等离子治疗舌根囊肿临床疗效观察

摘　　要：目的探讨显微喉镜下低温等离子治疗舌根囊肿的临床疗效。方法在全身麻醉气管插管条件下对14例舌根囊肿患者[囊肿大小(0.3 cm×0.4 cm)^(2.0 cm×2.0 cm)]采取显微喉镜下配合低温等离子治疗观察疗效,并进行随访。结果 14例舌根囊肿患者均一次切除成功,平均手术时间为10~20 min,出血量约5~10 ml。病理检查均为粘液囊肿。手术后数天内部分患者有轻微咽部不适、疼痛、吞咽困难现象,10 d后症状消除。术后1~2周舌根伪膜脱落,表面愈合良好。随访3~6个月,舌根黏膜光滑、无复发,无舌根周围组织损伤,无伸舌困难,无其他并发症。总有效率、手术时间、出血量均获满意,语音和舌根运动功能不受影响。结论显微喉镜配合低温等离子治疗舌根囊肿具有术野清晰、照明度好,快速、出血少、安全、切除完全、复发率低和患者痛苦小等优点,值得临床推广应用。Objective To explore curative effect of low-temperature plasma radiofrequency ablation by microlaryngoscopic in treatment of tongue base cyst. Methods Low-temperature plasma radiofrequency ablation by microlaryngoscopic was used to treat 14 patients with the cyst of tongue base under general anesthesia with dndotracheal intuhation [ cyst size （0.3 cm x 0.4 cm ） - （ 2.0 cm x 2.0 cm） ]. Clinical curative effect was recorded and follow-up was conducted after surgery. Results The tongue base cysts were totally resected at one time in 14 patients, the mean operative time was 10 to 20 min, bleeding volume was about 5 - 10 ml, pathology indicated mucocele. A few of patients felt slightly throat irritation, sore, difficulty swallowing after several days, symp- toms were resolved in l0 days. Pseudomemhrane of root of tongue fell off after 1 - 2 weeks, surgical trauma healed perfectly. The tongue mucosas were smooth without surround tissue injury and other syndromes. The total effective rate, operating time and blood loss was satisfied at follow-up of 3 - 6 months. Speech and the motor function of tongue were not affected. Conclusion Low-tem- perature plasma radiofrequency ablation by microlaryngoscopic was worthy to promote with clear operative area, minimally invasive ,little blood loss, safety and low recurrence rate.
