摘  要:目的溃疡性结肠炎的发病机制目前并未完全明确。调节性T细胞是一组具有免疫抑制作用的T细胞亚群,研究显示其与溃疡性结肠炎的发病具有密切相关性。观察溃疡性结肠炎(ulcerative colitis,UC)外周血CD4+CD25+CD127low/-调节性T细胞(Tregs)及IL-10的表达水平,探讨其在UC发病机制中的作用。方法纳入UC患者53名,其中轻度34名,中度19名,健康体检者20名作为对照组,以流式细胞术检测外周血CD4+CD25+CD127low/-Treg,放射免疫检测IL-10。结果两组UC患者外周血中CD4+T细胞百分比、CD4+CD25+CD127low/-Treg细胞占CD4+T细胞百分比及血清IL-10表达水平均低于正常对照组(P﹤0.05),且病情中重度者更加明显(P<0.01)。结论 UC患者CD4+CD25+CD127low/-Treg,IL-10表达水平减低,说明其可能参与了UC的发病,可以临床诊断及治疗提供依据。Objective The pathogenesis of ulcerative colitis was not fully clear. Regulatory T cells was a group of T - cell subsots with immunosuppressive effects. Studies have shown its close correlation with the incidence of ulcerative colitis. This paper aims at discussing the role of CD4^+CD25^+CD127^low/- regulatory T cells (Tregs) and IL - 10 expression levels in UC pathogenesis through observation on them in peripheral blood of ulcerative colitis (UC) patients. Methods Fifty -three UC patients, including 34 mild cases and 19 moderate cases, and 20 healthy individuals as a control group, were involved to detect CD4^+CD25^+CD127^low/-Treg in peripheral blood by flow cytometry, and radioimmunoassay was employed to detect the IL- 10 level.Results The CD4^ - cell percentage, percentage of CD4^+CD25^+CD127^low/-Treg in CD4^ T - cell and serum IL - 10 expression levels in peripheral blood of UC patients were all lower than those in control group (P 〈 0.05 ), showing positive correlation with the severity of UC ( P 〈 0.01 ). Conclusion The decrease of CD4^+CD25^+CD127^low/-Treg and IL - 10 expression levels maybe involved in the pathogenesis of UC, and it can provide the basis for clinical diagnosis and treatment.
