四种恶性肿瘤患者外周血T淋巴细胞亚群分析

摘　　要：目的探讨四种恶性肿瘤患者外周血T淋巴细胞亚群及NK细胞的变化及其临床意义。方法采用流式细胞术检测234例四种恶性肿瘤患者及45例健康体检者外周血T淋巴细胞亚群、NK细胞及B细胞。结果与对照组相比,四种恶性肿瘤患者总T淋巴细胞比例有降低趋势,但差异无统计学意义(P>0.05),CD4+T淋巴细胞占总淋巴细胞的百分比降低(P<0.05),CD8+T淋巴细胞和NK细胞占总淋巴细胞的百分比升高(P<0.05);乳腺癌和肺癌患者CD8+T淋巴细胞占总淋巴细胞的百分比高于胃癌和结肠直肠癌患者,而CD4+T淋巴细胞占总淋巴细胞的百分比低于胃癌和结肠直肠癌患者(P<0.05);。结论恶性肿瘤患者整体免疫功能低下,同时呈现免疫紊乱状态;乳腺癌和肺癌患者免疫功能改变与胃癌和直肠结肠癌略有不同,可能与肿瘤的侵袭、转移有关。Objective To explore the changes and clinical significance of T - lymphocyte subgroups and NK cells in peripheral blood of four types of cancer patients. Methods T - lymphocyte subgroups, NK cells and B cells were detected in 234 cancer patients and 45 healthy controls by flow cytometry. Results Compared with normal control group, decrease was observed in total T - lymphocyte cells in four types of caner patients, without statistical difference （ P 〉 0.05 ） ; In total T - lymphocyte cells, CD4 ^＋ T - lymphocyte cells decreased, CD8 ^＋ T - lymphocyte cells and NK cells increased, with significant statistical difference （P 〈 0.05） ; CD8^＋T - lymphocyte cells were higher in breast cancer and lung cancer compared with gastric cancer and colon cancer （P 〈 0.05 ）, while CD4^＋ T -lymphocyte cells were lower compared with gastric cancer and colon cancer （P 〈 0.05 ）. Conclusion Low immune function and immune dysfunction are observed in cancer patients. Changes of immune function were different in breast cancer and lung cancer compared with gastric cancer and colon cancer, it may be related with tumor invasion and metastasis.
