长沙市中心医院815例住院青光眼患者构成及临床研究

摘　　要：目的探索基层医院青光眼更好的早期防治模式。方法对长沙市中心医院2010年1月至2012年12月住院的青光眼患者815例进行统计学分析。分析长沙地区青光眼住院患者的类型构成、性别、年龄分布及其相关因素。结果 815例患者中原发性青光眼572例,占70.18%,其中以闭角型为主,共560例,占97.90%;开角型12例,仅占2.10%。继发性青光眼236例,占28.95%,先天性青光眼7例,占0.86%。原发性闭角型青光眼中女性患者比例明显高于男性,而继发性和原发性开角型青光眼男性患者比例则显著高于女性,差异有统计学意义(P<0.05)。结论长沙地区青光眼住院患者患病有明显的性别、年龄相关性,构成以原发性闭角型青光眼为主要类型,表明PACG仍然是基层医院眼科青光眼防治的重点,另外,我们对于POAG的防治,应注意变化的病因及临床特征,重点加强社区宣传,增加机会性筛查等。Objective To investigate the better early prevention and control model of glaucoma in basic hospital. Methods Statistical analysis was conducted for 815 inpatients with glaucoma in Changsha central hospital between January 2010 and December 2012. The proportion of difference types, distribution of genders, ages, related factors of glaucoma inpatients in Changsha area were analyzed. Results In all of the 815 eases, there were 572 patients （70. 18% ） with primary glaucoma （PC,）. In PG, primary angle-colsure glaucoma （PACG） was the main type, accounted for 97.90% （560 cases） ; primary open angle glaucoma （POAG） only accounted for 2. 10% （12 eases）. There were 236 patients （28.95%） with secondary glaucoma （ sa）, 7 patients（0. 8% ） with congenital glaucoma. The ratio of female was obviously higher than the ratio of male in PACG pa- tients, the ratio of male was obviously higher than the ratio of female in SG and POAG patients, there were significant differences （P 〈 0. 05 ）. Conclusion The morbility of glaucoma is apparently related with sexualiy, age in hospitalized patients in Chang- sha area. PACG is still the major type of glaucoma. The prevention and cure of PACG is the focal point in department of ophthal- mology in hospital. Moreover, we should take attention to the etiological factor and changed clinical feature of POAG, emphasize on publicizing in community, and increase the opportunity screening.
