Discussion on the Hemodynamic Impact of CAR on Severely-burned New Zealand Rabbits During Their Shock Phase

摘  要:目的 探讨卡巴胆碱(CAR)对重度烧伤新西兰兔休克期血流动力学的影响.方法 普通级雄性成年新西兰兔12只,经右颈总动脉和主动脉插管至左心室,连接压力换能器将信号输入到多功能生理记录仪.使用电烫伤仪制成30%Ⅲ度烧伤模型.随机分为对照组(A组)和实验组(B组)(n=6).制模完成后向对照组和实验组胃内分别注入葡萄糖-电解质溶液(GES)和等量含CAR的GES(20μg/kgCAR溶于GES),伤后1 h起静脉补液.测定非麻醉状态下两组兔心率(HR)、左心室收缩压(LVs)、平均动脉压(MAP)、左心室舒张末压(LVEDP).结果 烧伤后对照组和实验组兔的HR、LVs、MAP、LVEDP分别出现相应改变,30 min内两组兔的测定数据间差异无统计学意义.烧伤后1h实验组较对照组HR下降12.86%,烧伤后2h、4h分别为19.61%和21.39%,差异有统计学意义(P<0.05),且HR趋于稳定.烧伤后2h、4h,实验组较对照组LVs上升分别为35.11%和41.17%,MAP升高分别为47.35%和58.07%,差异均有统计学意义(P<0.05).烧伤后1h实验组较对照组LVEDP下降31.85%,差异有统计学意义(P<0.05),烧伤后2~4 h差异无统计学意义(P>0.05).结论 CAR够改善烧伤休克期新西兰兔的HR、LVs、MAP、LVEDP,改善心脏收缩和舒张功能.Objective To discuss the hemodynamic impact of CAR on severely-burned New Zealand rabbits during their shock phase. Methods 12 adult male New Zealand rabbits of ordinary grade were used in the experiment. A tube is inserted from the rabbits' RCCA (right common carotid artery) and aorta to their left ventricle, to which the press transducer was connected to transmit the signals into muhifunctional electrophysiolograph. Create moulds of 30% three-degree burned by electric scald apparatus. A stochastic grouping method was introduced for control group (Group A) and experimental group (Group B) (n=6). After molding, inject GES and the same amount of GES with CAR (20 tt g/kgCAR in GES) into the stomachs of the control group and experimental group respectively, and supply intravenous fluid one hour after burned. Measure the HR/ LVs/MAP and LVEDP of all the rabbits in experiment under the non-anesthesia condition. Results After burned, rabbits' degrees of HR/ LVs/ MAP/LVEDP in the control group and experimental group show corresponding alteration, yet no striking differences appeared between the two groups within 30 minutes after burning. One hour after burned, in comparison to rabbits in control group, the rabbits from experimental group experienced a decrease of 12.86% in their HR, and two hours and four hours after burned the results were 19.61% and 21.39%. The distinctions are striking here (P〈O.05), and the HR reached stable. Two and four hours after burned, in comparison to rabbits in control group, the LVs of those in experimental group witnessed an increase of 35.11% and 41.17%, while the level of MAP increased by 47.35% and 58.07%. Judging from this, the distinction is significant (P〈0.05). As regard to LVEDP, it decreased by 31.85% one hour after burned with a obvious distinction (P〈0.05), and two and four hours after burned with no obvious distinction (P〉0.05).
