国产阿托伐他汀与立普妥治疗颈动脉粥样硬化斑块的疗效及安全性评价

摘　　要：目的:通过比较国产阿托伐他汀(阿乐)与立普妥治疗颈动脉粥样硬化斑块的疗效及安全性。方法:颈动脉粥样硬化患者157人,随机分为三组,A组(51人)、B组(49人)及C组(57人),分别采用立普妥10mg、立普妥20mg及阿乐10mg进行治疗,在治疗开始前、治疗24周时对患者的血脂(LDL-C及TC)、双侧颈总动脉(CCA)、双侧颈动脉分叉(BULB)及颈内动脉(ICA)处斑块厚度(IMT)进行检测,并对不良反应发生的情况进行分析。结果:三组治疗前LDL-C和TC无差异,治疗后与治疗前相比均有不同程度的降低,治疗后B组降低血脂的效果优于其余两组(P<0.05)。治疗24周后,CCA和BULB的IMT有明显改善(P<0.01),ICA的IMT无明显改善(P>0.05)。治疗后三组组间比较IMT均无统计学差异(P>0.05)。结论:立普妥和阿乐均具有明显的降低血脂和改善IMT作用,不良反应发生率也较低。Objective：Evaluation of efficacy and safety of Domestic Atorvastatin and Lipitor on Carotid Atherosclerotic Plaques. Methods：Patients were divided randomly into three groups：group A（n=51）,group B（n=47）,group C（n=57）. Lipitor 10,20mg and ALe 10 mg were used for treatment on thoses groups respectively. Blood lipid（LDL-C, TC）, bilateral common carotid artery（CCA）, bilateral carotid artery bifurcation（BULB）, internal carotid artery（ICA） and in plaque thickness （IMT） were detected before the treatment and after 24 weeks of treatment. And the adverse reactions were analyzed. Results：LDL-C and TC had no significant difference between three groups before the treatment,After treatment for 24 weeks, the levels of blood lipid was lower than before the treatment , The levels of blood lipid of group B was lower than the other groups（P〈0.05）. After the treatment for 24 weeks,CCA and BULB IMT had significantly improved（P〈0.01）,there was no significant diference in the internal carotid artery IMT（P〈0.05）,there was no significant differences of IMT among the three groups（P〈0.05）. Conclusion：Lipitor and ALe were both reduced the blood lipid level and improved IMT effect, the incidence of adverse reactions was low either.
