摘  要:目的探讨急性脑梗死(acute cerebral infarction,ACI)患者血清脂联素、过氧化物酶体增殖物活化受体γ(peroxisome proliferator-activated receptor gamma,PPARγ)、氧化型低密度脂蛋白(oxidized low density lipoprotein,ox-LDL)及单核细胞CD36、CD54表达的特点,并分析其相关性。方法选择ACI患者(ACI组)31例、健康体检者(对照组)27例,检测血脂,ELISA方法测定血清脂联素、PPARγ、ox-LDL浓度,流式细胞技术检测外周静脉血单核细胞CD36、CD54表达。结果 ACI组血清脂联素、PPARγ、HDL-C低于对照组,ox-LDL、CD36、CD54、TG、LDL-C高于对照组(均P<0.01)。相关分析显示,血清脂联素水平与PPARγ、ox-LDL呈正相关(r=0.939、0.934,P<0.01),PPARγ与ox-LDL呈正相关(r=0.945,P<0.01),CD36与CD54呈正相关(r=0.760,P<0.01)。结论 ACI患者血清脂联素、PPARγ降低,ox-LDL、CD36、CD54升高,血脂异常,其相互网络调控是相互促进、相互制约的,在ACI发生发展中起重要作用。Objective To Observe the serum level of adiponectin, peroxisome proliferator- activated receptor gamma (PPAR~/), oxidized low density lipoprotein (ox-LDL) and CD36, CD54 expression in monocytes of patients with acute cerebral infarction (ACI), and analyze their correlation. Methods Lipids were measured in 31 patients with ACI (ACI group)and 27 healthy subjects (control group), serum concentrations of adiponeetin, PPAR-γ and ox-LDL were measured by using ELISA, CD36, CD54 expression in monocytes were measured by using flow cytometry, and the corelation analysis was done between them. Results Compared to the control group, serum concentration of adiponectin and PPARγ, HDL-C decreased, while ox-LDL, CD36, CD54, TG, LDL-C increased in ACI group ( all P 〈0.01 ). Correlation analysis showed that serum concentration of adiponectin was positively corelated with PPARγand ox-LDL (r =0.939,0. 934,0. 945 ,all P 〈0.01 ),and CD36 was positively corelated with CD54( r = 0. 760 ,P 〈 0.01 ). Conclusion In patients with ACI, the serum levels of adiponectin, PPARγ were decreased, CD36, CD54 expression in monocytes were increased, dyslipidemia, adiponectin, PPARγ, ox-LDL, CD36, and CD54 expression of monocyte may influence mutually and play an important role in the occurrence and development of ACI.
