摘  要:目的检测CD56、CK19、galectin3在甲状腺病变组织中的表达情况,探讨CD56、CK19、galectin3的表达在甲状腺病变鉴别诊断中的作用。方法收集海南医学院附属医院病理科71例甲状腺腺癌、64例良性病变组织蜡块。将135例石蜡标本制做组织芯片。应用免疫组织化学技术检测CD56、CK19、galectin3蛋白在甲状腺病变组织中的表达情况。结果 CD56在甲状腺腺癌组织中大多表达缺失,表达率为12.7%,在良性甲状腺病变组织中表达率高(92.2%),两者间表达具有显著的差异(P<0.01)。CK19、galectin3在甲状腺腺癌组织中表达较高(88.7%,93.0%),呈弥漫强阳性表达;在良性甲状腺组织中表达较低(17.2%,7.8%),主要呈散在弱阳性表达;两者间的表达具有显著性差异(P<0.01)。同一组织中,CD56表达缺失、CK19、galectin3表达增高的腺癌组织有59例(83.1%),良性病变0例,两者间差异明显(P<0.01)。结论 CD56在甲状腺腺癌组织中表达缺失,在甲状腺良性病变组织中表达增高,对甲状腺腺癌的诊断具有一定的辅助意义;CD56表达缺失结合CK19、galectin3的高表达对甲状腺腺癌的鉴别诊断具有重要作用。Abstract:Objective To detect the expression level of CD56,CK19,galectin3 in different thyroid lesions,and discuss the application of CD56,CK19,galeetin3 for the differential diagnosis of thyroid lesions. Methods Paraffin samples of 71 cases with thyroid carcinoma,64 eases thyroid benign hyperplasia tissues were collected in Pathology department of Affiliated Hospital of Hainan Medical College and the expression level of CD56,CK19,galeetin3 in thyroid lesions were detected by Immunohistochemieal techniques. Results The expression level of CD56 was deficit in most thyroid carcinoma with a positive rate of 12.7%, the expression level of benign tissue was higher than that of thyroid carcinoma (92.2%vs12.7% ,P〈0.01). CK19,the positive rate of galectin3 was higher in thyroid carcinoma than in benign thyroid (88.7%vs17.2%and 93.0%vs7.8% ,P〈0.01 ). There were 59 cases thyroid carcinoma with CD56 expression deficit and enhanced expression of CK19 and galectin3 (P〈O.O1). Conclusions The expression level of CD56 was deficit in thyroid carcinoma and enhanced in benign thyroid lesions,Thus it is of subsidiary significance in differential diagnosis of thyroid careinomar.
