帝视内镜与直接喉镜对牙齿畸形患者气管插管并发症的影响观察

摘　　要：目的观察帝视内镜和直接喉镜气管内插管对牙齿畸形患者全麻气管插管时并发症及血压、心率的影响。方法选择术前ASA分级为I～Ⅱ级、无困难气道、有牙齿畸形的患者60例，随机分为内镜组（D组）和喉镜组（M组），全麻常规诱导后，两组患者分别用帝视内镜或普通喉镜插入气管导管，观察并记录两组患者气管插管并发症发生率、插管成功率以及插管前、插管时、插管后1minMAP和HR的变化。结果D组插管并发症（牙齿松动脱落、口内黏膜出血、术后咽喉痛）明显少于M组（P〈0．05）。两组间插管成功率差异无统计学意义（P〉0．05）；气管插管时M组MAP及HR值明显高于插管前（P〈0．05），且高于D组（P〈0．05），插管后1min两组MAP及HR值均高于插管前（P〈0．05），但组间差异无统计学意义（P〉0．05）。结论帝视内镜下牙齿畸形患者行气管插管并发症少，咽喉部刺激损伤轻，血流动力学较稳定。Objective To compare the impact of discopo and direct laryngoscope trachea intubation on complications, blood pressure and heart rate of teeth deformity patients under general anesthesia. Methods 60 patients with pre-op- eration ASA grade I - II, no difficult airway, but suffering teeth deformity were selected, then they were randomly divided into two groups： endoscope group （group D） and laryngoscope group （group M）. After routine induction of general anesthesia, patients in the two groups were conducted endotracheal intubation with discopo or common laryngoscope respectively. The occurrence rate of complications, the success rate of intubation, as well as the change of MAP and HR before intubation, during intubation and one minutes after intubation were recorded and compared. Results The complications （gomphiasis and tooth loss, mucosal hemorrhage in mouth, post-operation pharyngalgia） of intubation in group D was less than that of group M （P〈0.05）, and the success rate of intubation in two groups was not significantly different; during intubation, the MAP and HR value in group M was apparently higher than that before intubation （P〈 0.05）, and was also higher than group D （P〈0.05）; 1 min after intubation, MAP and HR in the two groups were both higher than that before intubation （P〈0.05）, but there was no significant difference between two groups（P〉0.05）. Conclusion Discopo assisted intubation in patients suffering teeth deformity has lower occurrence rate of complication,also lower stimulation and injury to the throat, with more stable haemodynamics state.
