癫痫患者的疾病经济负担及相关影响因素研究

摘　　要：目的：探讨癫痫患者疾病的经济负担及其相关影响因素。方法：对120例癫痴患者进行问卷调查，了解患者及其家庭的基本资料和2008年2月至2009年2月期间有关癫痫的费用支出。应用多元线性逐步回归分析判断影响癫痫患者疾病经济负担的相关因素。结果：癫痫患者的年平均有关治病的支出为11063．45元，其中直接费用为8758．25元，间接费用为2305．20元；占户均家庭收入的23．2％。多元逐步回归分析显示，癫痫患者是否住院（t=-8．194，P=0．000）、抗癫痫药（AED）类别（t=3．365，P=0．001）、发作频率（t=2．956，P=0．004）、居住地（t=2．078，P=0．040）为影响癫痫经济负担的相关因素。结论：癫痫患者的疾病经济负担较重。本次调查还发现，因羞耻感而不愿或拒绝通过新农合、城镇医保等医疗保障途径解决经济负担的患者有12例（10．8％），说明政府和社会对癫痫和癫痫病人的关注度还有进一步提升的空间。Objective：To explore epilepsy patients＇ medical expenses and their factors of influence. Methods：The data were collected via questionair surveys on 120 patients with epilepsy,in which patients were questioned about basic demographics information and medical expenses from February 2008 to Feb- ruary 2009. Then mutivariate stepwise regression methods were used to explore potential factors of IN FLUENCEON epilepsy patients＇ medical expenses. Results：On the average, an epilepitc patient＇s ex- penses were RMB￥ 11063.45, including direct expense Y8758. 25 and indirect expenses Y2305.20, taking up. 23.2 percents of family income. Multivariate stepwise regression analysis showed that factors influencing epileptic patients＇ medical expense included：whether the patient received hospital treatment （t=-8. 194, P=0. 000）, anti-epilepsy-drug （AED） category（t=3. 365, P=0. 001）, disease seizure frequency（t= 2. 956,P = 0. 004）and local financial expense factors （t= 2. 078,P= 0. 040）. Conclusion： Patients with epilepsy have heavier medical expense burden. The analysis also shows 12 of the subjects （10.8 percents） are reluctant or refuse to solve their issues of medical expenses via New Rural Credit Union or Urban Medical Insurance Plan because they feel shameful about it. This indicates that the government and society should pay more attention to the disease of epilepsy epilepsy and the epileptic patients.
