摘 要:目的研究胰岛素样生长因子I(insulin-like growth factor-I,IGF-I),胰岛素样生长因子I型受体(insulin-like growth factor-I receptor,IGF-IR),雌激素受体α(estrogen receptorα,ERα)在卵巢癌中的表达及其与临床病理特征的关系。方法于2007年3月至2008年10月间进行病例对照研究,病例组为132例手术后病理确诊的原发性卵巢癌患者,对照组为40例因妇科良性肿瘤行手术治疗的患者,收集手术切除的卵巢癌或正常卵巢组织,抽提总RNA,Real-time PCR法定量分析IGF-I、IGF-IR及ERα的表达。结果 IGF-I、IGF-IR和ERα在卵巢癌组织中的表达量显著升高;卵巢癌病期越晚、组织分化越差,IGF-I、IGF-IR的表达越高;术后有残余病灶的患者,IGF-IR的表达升高。卵巢癌组织中IGF-I与IGF-IR的表达呈正相关(r=0.35,P<0.001),IGF-I、IGF-IR与ERα的表达无相关性。结论卵巢癌局部组织中IGF-I/IGF-IR形成的自分泌、旁分泌系统在卵巢癌发生发展中发挥重要作用。绝经前女性体内雌激素与IGF系统之间可能有协同作用。组织IGF-I、IGF-IR的检测,对判断卵巢癌病情变化有一定临床价值。Objective: We investigated the role for the insulin -like growth factors (IGFs), their receptor, IGF -IR, and Estrogen receptor a (ER(x) in the induction and progression of ovarian cancer. Methods : A case - control study conducted in the First Affiliated Hospital of Harbin Medical University in China, between 2007 and 2008. Fresh specimens were collected from cases with histologically confirmed ovarian cancer and matched controls who underwent surgery for benign diseases. IGF - I, IGF - IR, and ERα expressions were analyzed using quantitative real - time PCR. Results : High expression of IGF - I and IGF - IR was associated with elevated risk of ovarian cancer. The association was dose - dependent and was more evident in premenopausal women than in postmenopa- usal women. Both IGF - I and IGF - IR expression were found to be higher in tumors with poor prognosis. Patients with either suboptimal debulking or with residual tumor after surgery had slightly higher IGF - IR expression. Intratumoral IGF - I expression was posi- tively correlated with the expression of IGF - IR, but not with Erα. Conclusions : The increased intratumoral IGF - I and IGF - IR expression suggests an involvement of the IGF - I/IGF - IR axis in the biological behavior of ovarian cancers in this population and could define a potential therapeutic target.
