神经质人格、述情障碍对中老年消化性溃疡病人抑郁症状躯体化的因素相关影响分析

摘　　要：目的：探讨中老年消化性溃疡病人群体的抑郁症状躯体化水平以及神经质人格、述情障碍对于消化性溃疡的影响，为临床心理治疗提供参考。方法抽取我市三所三级医院、一所二级医院、一所部队医院门诊消化性溃疡患者535人进行流调中心用抑郁量表、多伦多述情障碍量表和大五人格问卷-神经质分量表调查，同时，对呼吸科门诊上呼吸道感染患者225人进行流调中心用抑郁量表问卷调查，做为对照组对比分析。结果溃疡组抑郁症状、积极情绪、躯体症状、人际分量表的分值高于非溃疡组，消化性溃疡女性患者组的躯体症状、缺乏情感识别、缺乏情感描述和神经质人格的分值高于男性组，消化性溃疡中年患者组的缺乏情感描述、躯体症状、神经质人格的分值低于老年患者组（P＜0.01）；躯体症状和述情障碍、神经质人格呈正相关；神经质人格和述情障碍成正相关（P＜0.01）；缺乏情感描述对躯体症状有直接正向影响，神经质人格在缺乏情感识别、外向性思维、缺乏情感描述和躯体症状中起到中介效应（X2/df=1.253， p=0.315，RMSEA=0.019，NFI=0.967，RFI=0.924，IFI=0.957，TLI=0.965，CFI=0.937）。结论中老年群体消化性溃疡患者的抑郁症状躯体化水平较高，神经质人格在消化性溃疡患者的述情障碍和抑郁症状躯体化之间呈中介效应。Objective By exploring the inlfuences of neuroticism and alexithymia of elderly patients with deptic ulcer on their Somatization, to provide references for mental health education. Method Set the535 recruits elderly patients with deptic ulcer of four hospital as the objectives, and investigate them with the methods of TAS-20、FFI-N、CED-S. 225 elderly patients with upper respiratory tract were chosen to form the constrast group. Result The total scale score of TCED-S and each sub-scale were different on deptic ulcer patients and upper respiratory tract patients . The sub-scale score of TAS-20、FFI-N、CED-S different on military ages and sex deptic ulcer patients signiifcantly （ P〈0.01）.The scale of depressive somatic symptomos was positively correlated with the scores of TAS-20、FFI-N. the scale of TAS-20 was positively correlated with the scores of FFI-N（ P〈0.01）. The patients′dififculty describing feelings directly related to related to depressive somatic symptomos of patients. The patients′ neuroticism mediate the relationship among dififculty identifying feelings and dififculty describing feelings and externally oriented cognitive style of thinking of patients.The model ift indexes of were X2/df=1.253,p=0.315,RMSEA=0.019,NFI=0.967,RFI=0.924,IFI=0.957,TLI= 0.965,CFI=0. 937. Conclusion Deptic ulcer patients represented obvious depressive somatic symptomos. neuroticism mediate the relationship among dififculty identifying feelings and dififculty describing feelings and externally oriented cognitive style of thinking.
