宫颈癌筛查影响因素的探讨

摘　　要：目的 分析不同年龄段及细胞学检测结果不同患者的人乳头瘤病毒（HPV）感染率,探讨宫颈癌筛查的理想方案.方法 对郑州人民医院病理科2012年1月至2012年12月自愿接受宫颈癌筛查的1397例患者进行宫颈液基细胞学（TCT）的病理诊断及HPV感染的检测,然后将所有病例分为6个不同年龄组进行TCT结果和HPV感染率的分析,最后采用x2检验对这些计数资料进行统计学分析.结果 TCT检测结果为不明确意义的不典型鳞状上皮细胞（ASC-US）及以上的患者HPV感染率明显高于TCT检测正常及炎症的患者,但是TCT检测正常及炎症的患者,HPV感染率也达到了20％左右.6个不同的年龄组HPV感染率比较差异有统计学意义,其中＜20岁及≥60岁的患者HPV感染率都达到了25％以上.结论 性生活存在与否及年龄因素是影响HPV感染率的两个重要因素,将TCT检测、HPV-DNA检测与年龄因素及性生活存在与否有效结合起来,能早发现、早治疗早期病变,最大限度地降低宫颈癌的病死率.Objective To analyze the HPV infection of different age groups and cytology test results cases,and investigate the ideal method of cervical cancer screening.Methods A total of 1397 patients who received cervical cancer screening voluntarily in department of pathology in the people＇s hospital of Zhengzhe from January 2012 to December 2012 received TCT detection and HPV-DNA detection and were divided into 6 different age groups,then the results of TCT detection and the infection rate of HPV were analyzed respectively.All data were statistically analyzed using chi-square test.Results The HPV infection rate of the patients with atypical squamous cells of undetermined significance （ASC-US） or more serious by TCT detection was significantly higher than the HPV infection rate of those cases with normal TCT detection result and inflammation.But the HPV infection rate of the normal and inflammatory cases has reached more than 20％.There was significant difference in HPV infection rate among the 6 different age group,and the HPV infection rate had reached more than 25％ both in cases ＜20 years and ≥ 60 years.Conclusions Sexual life and age are two important factors that affecting the infection rate of HPV,so effectively joint TCT detection and HPV-DNA detection with sexual life and age,can find early cervical lesions timely and reduce the mortality induced by cervical cancer.
