脑清喷鼻微乳治疗急性缺血性脑卒中100例随机对照临床研究

摘　　要：目的:观察脑清喷鼻微乳治疗急性缺血性脑卒中的临床疗效。方法:共100例急性缺血性脑卒中患者纳入研究,采用随机数字表法分为脑清喷鼻微乳组(治疗组)与空白安慰剂组(对照组)各50例。两组患者均采用常规基础治疗,治疗组予脑清喷鼻微乳喷鼻,2喷/次,3次/d;对照组予不含药物成分的微乳,2喷/次,3次/d。以美国国立卫生研究院卒中量表(NIHSS)、Fugl-Meyer运动功能评定量表、生活能力状态表、中风症状分级量化表及血液流变学检查评估两组患者治疗治疗前与治疗后7天、14天的愈显率及有效率。结果:治疗组总有效率93.33%,愈显率60.00%,其愈显率明显高于对照组的42.00%(P<0.05)。在NIHSS评分、Fugl-Meyer运动功能评定量表、日常生活能力量表积分、中医症候积分及血液流变学改善方面均明显优于对照组(P<0.05)。结论:脑清喷鼻微乳经鼻给药治疗急性缺血性脑卒中疗效肯定,可改善患者的神经功能状况,中医症状及血流变指标,提高其日常生活能力,为临床治疗提供依据。Objective ： To evaluate the therapeutic effect of Naoqing Penbi Weiru combined with urokinase on acute cerebral in farction. Methods ： All the 100 qualified patients were grouped randomly into Naoqing Penbi Weiru group （ treatment group） and blank group （ control group）. Every group had 50 patients. Two groups of patients were all treated with conventional treatment. The treatment group was treated by Naoqing Penbi Weiru （two spurt one time, three times a day） ,while the control group was treated by blank Penbi Weiru （two spurt one time, three times a day）. The efficiency of two groups was assessed respectively, recordingthe NIHSS scores, Fugl Meyer scores, ADL scores, the classification of acute cerebral infarction symptom scores, the hemorheolo gy and the head CT of two groups of patients before and 7 days and 14 days after treatment. Results ： The effective rate of treatment group was 93.33% and the show rate was 60.00% which was higher than that of the control group 42.00% （P 〈 0. 05）. The im provement of NIHSS scores, the improvement of Fugl Meyer scores, ADL scores, the classification of acute cerebral infarction symptom scores and the hemorheology were better than those of the control group （ P 〈 0.05 ）. The head CT had the better trend of treatment group. Conclusion：The Naoqing Penbi Weiru method combined with urokinase can improve the neural function,the abili ty of daily life, the classification of acute cerebral infarction symptom and the hemorheology, avoiding the area of acute cerebral in farction amplifying effectively. The study could offer the evidence of clinical treatment.
