胰腺内分泌肿瘤23例临床诊治分析

摘　　要：目的:总结胰腺内分泌肿瘤的临床特点和治疗经验。方法:回顾性分析中国医科大学附属盛京医院2003年7月-2011年7月收治并经病理确诊的23例胰腺内分泌肿瘤患者的临床资料,其中胃泌素瘤1例,胰岛素瘤22例。结果:1例胃泌素瘤患者因血清胃泌素明显升高而得到定性诊断,22例胰岛素瘤患者中17例因典型的"Whipple三联征",胰岛素释放指数>0.3而获得定性诊断。联合应用腹部彩超、胰腺增强CT、超声内镜检查,全部患者均得到准确的定位诊断。所有患者均行手术治疗,除1例术前伴有哮喘的患者术后出现呼吸衰竭死亡外,其余患者随访远期效果均较良好。结论:血清学相关激素水平的测定对于胰腺内分泌肿瘤的诊断必不可少,超声内镜诊断胰腺内分泌肿瘤具有一定的优越性。手术切除是治疗胰腺内分泌肿瘤最主要的手段,局限浸润和肝转移并不是手术禁忌症。Objective：To summarize the diagnosis and treatment of pancreatic endocrine tumors. Methods：All 23 cases of pancreatic endocrine tumors admitted to Shengjing Hospital of China Medical Univerisity during July,2003 - July,2011 were analysized retrospectively, which included 1 case of gastrinoma and 22 cases of insulinoma. Results ： 1 case of gastrinoma was diagnosed because of significantly elevated level of serum gastrin, 17 cases of insulinoma had typical ＂Whipple triad＂ with IRI 〉 0.3, which were diagnosed easily. Operations were performed among all the pa- tients. After a long time of follow - up, all the patients had good outcome but one patient with asthma pre - operation acquired respiratory failure and died after the operation. Conelusion： Measurements of special serum hormone are es- sential for the diagnosis of pancreatic endocrine tumors. EUS has some advantages in the diagnosis of pancreatic endo- crine tumors. Surgical resection is the most important way to treat these tumors, and local invasion or live metastases is not the contraindication of surgery.
