摘　　要：目的总结胆管癌18F-FDG PET/CT显像表现,提高胆管癌的诊断准确性。方法回顾经病理或临床综合手段证实的53例胆道疾病的18F-FDG PET/CT表现,分析PET/CT诊断胆管癌的敏感性、特异性和准确性。结果肝内胆管癌14例、近段胆管癌18例、中远段胆总管癌15例、胆管炎性病变或伴结石6例。肝内转移9例,腹腔及腹膜后淋巴结转移15例,椎体等远处转移3例。PET/CT诊断胆管癌的敏感性为95.7%、特异性为83.3%、准确性为94.3%。结论 18F-FDG PET/CT在胆管癌的诊断与鉴别诊断、分期、检测疗效及预后等具有独特的应用价值。Objective To summarize the lSF-FDG PET/CT imaging features of bile duct carcinoma, and to improve the accuracy of diagnosis. Methods We reviewed 53 cases of performance confirmed by ISF-FDG Whole imaging or regional imaging by means of pathology or clinic and analysed the sensitivity, specificity and accuracy PET/CT diagnosis of cholan- giocarcinoma. Results 14 patients were intrahepatic cholangiocarcinoma. 18 patients were proximate biliary duct cancer （hilar cholangiocarcinoma）, 15 patients were extrahepatic biliary duct cance, 6 patients were choledocholithiasis or Cholangitis. 9 patients had intrahepatic metastasis, 15 patients had hepatic hilum, hepatodudenal ligament and retroperito- neal lymph node metastasis, and 3 patient had spine body or other metastasis. The sensitivity, specificity and accuracy for detection of cholangiocarcinoma of ~8 F-FDG PET-CT were 95.7 ~, 83.3 ~, 94.3 ~. Conclusion 18 F-FDG PET/CT plags playing an increasingly important role in the diagnosis and differential diagnosis of cholangiocarcinoma. It is also of a sig nificant practical value in grading, guiding therapy and prognosis of a variety of cancer types.
