阿尔茨海默病和轻度认知功能障碍患者的危险因素分析

摘　　要：目的探讨阿尔茨海默病(AD)和轻度认知功能障碍(MCI)患者的发病危险因素。方法连续收集本院神经内科住院的老年患者,分为AD组、MCI组和正常对照组,调查血管性和非血管性危险因素。结果单因素结果显示非血管性因素中,年龄、体力劳动为AD的危险因素,体力劳动、腰臀比为MCI的危险因素。血管性因素中,血清总胆固醇、低密度脂蛋白(LDL-C)、服用阿司匹林为AD的危险因素,LDL-C在MCI组和AD组中差异有显著性。多因素结果显示体力劳动为MCI和AD发病的独立危险因素。结论控制血脂、服用阿司匹林、减轻体重和腰围、增加脑力劳动均可减少和延缓AD的发病。Objective To explore the risk factors of AD and non-ischemic MCI, and to prevent occurrence and development of the diseases by controlling the risk factors. Method Elderly patients hospitalized in the department of neurology were continuously collected. After excluding patients with depression and cerebral ischeia, the recruited pa- tients were classified as three groups：AD group,MCI group and normal control group. We investigated the vascular and non vascular risk factors, and analyzed the data by single and multiple factor analysis. Result Single factor analysis showed that among vascular factors ,age and manual labor were the risk factors of AD ,manual labor and waist-to- hip ratio（WHIR） were the risk factors of MCI,and manual labor and Body Mass Index（BMI） affected the conversion of MCI to AD. Among vascular factors, serum total cholesterol,low density lipoprotein（ LDL-C ） , aspirin were the risk factors of AD. There was significant difference of low density lipoprotein between MCI group and AD groups. The resuits of multivariate analysis showed that manual labor was the independent risk factor of MCI and AD. Conclusion This study indicated that lowering blood lipids, aspirin, reducing weight and waistline, increasing mental labor can reduce the incidence and delay the progression of AD.
