摘 要:目的分析泸州地区妇女不同宫颈病变程度与人乳头瘤病毒(HPV)感染的关系。方法采用基因芯片技术对泸州地区783例妇女不同程度宫颈病变样本分为慢性宫颈炎、CINⅠ、CINⅡ、CINⅢ、乳头状瘤及宫颈癌6组,并行HPV基因分型检测。结果 783例宫颈病变样本的总感染率为54.79%(429/783),其中宫颈癌感染率84.07%(95/113),显著高于其余各组(P<0.05);慢性宫颈炎感染率为42.21%(168/398),显著低于除CINⅠ组以外的各组(P<0.05);乳头状瘤组感染率79.31%(23/29),显著高于慢性宫颈炎及CINⅠ组(P<0.05)。429例阳性患者样本中,高危感染率80.89%(374/429),其中乳头状瘤组感染率显著低于其余各组(P<0.05),宫颈癌感染率显著高于其余各组(P<0.05)。HPV阳性患者亚型分布中,仅高危型感染者乳头瘤样病变感染率显著低于其余各组(P<0.05),仅低危感染者乳头状瘤感染率显著高于其余各组(P<0.05)。结论泸州地区宫颈病变妇女高危HPV感染与宫颈癌及宫颈上皮内瘤变有关,并有随宫颈病变程度加重而升高的趋势。低危HPV感染为尖锐湿疣等良性性传播疾病的主要病原因子。Objective To fight against cervical cancer,we investigated the relationship between human papilloma virus (HPV) infection and women with cervical disease in Luzhou area. Methods A total of 783 samples of women with cervical disease were divided into 6 groups(Including chronic cervicitis,CIN I ,CIN lI ,CIN ~ ,Papilloma and carcinoma groups),and were tested by HPV genotyping technology. Results The total infection rate of HPV was 54.79% (429/783). HPV prevalence was 84.07%(95/113) in Cervical cancer group,and was significantly higher than all the other groups(P -~ 0.05). HPV prevalence was 42.21%(168/398) in chronic cervicitis group,and significantly lower than the other groups except CIN I (P 〈 0.05). Papilloma group infection rate was 79.31% (23/29),significantly higher than the groups of chronic cervicitis and CINI group(P 〈 0.05). In the total of 429 HPV positive samples,80.89%(347/429) were infected by high-risk HPV subtypes, in which,papilloma infection rate was significantly lower from other groups(P 〈 0.05),cervical cancer infection rate was significantly higher from groups (P 〈 0.05). Only HR-HPV infection rate in the papilloma group was significantly lower from other groups (P 〈 0.05),but significantly higher from other groups(P 〈 0.05) with only LR-HPV infection ones(P 〈 0.05). Conclusion HR-HPV infection rate is positively associated with different levels of cervical pathological changes in Luzhou area. The LR-HPV infection is the major risk factor for the cervix papilloma.
