摘  要:目的 :探讨用手术切除的癫痫病灶进行抗癫痫药物研究的可行性。方法 :将手术中切除大脑皮层病灶切成脑薄片 ,通过脑标本的降温通氧保存脑片活力 ,用低 Mg2 + 和 4-氨基吡啶 (4 - AP)灌流诱导脑片自发性癫痫样放电 ,用自行研制的多孔浴槽实现对同一脑标本进行 4种药物实验 ,观察在抗癫痫药前后自发放电波幅变化。结果 :4例癫痫病灶及 2例非癫痫脑组织 (对照组 )实施了药物敏感实验 ,常用的 4种抗癫痫药在低浓度时 (μmol/ L :苯妥英钠 10 0 ,苯巴比妥 10 0 0 ,安定 30 ,卡马西平 10 0 )对癫痫脑片自发放电的抑制作用较对非癫痫脑片的作用弱。药物浓度增大 3倍后 ,对两组脑片均产生明显抑制作用。结论 :癫痫病灶皮层脑片在体外可以用来观察药物的抗癫痫作用 ,癫痫病灶片对常用抗癫痫药物的敏感性较非癫痫者差。Objectives:To know the feasibility of using epileptic foci excised from patients in an in vitro model of sensitivity test to anticonvulsants.Methods:The epileptic foci excised from patients were cut into thin slices.The viability of the brain tissue samples was preserved by low temperature and oxygen.Spontaneous epileptic discharges were induced by perfusion with low Mg 2+ solution containing 4-AP aminopyridine 20 μmol/L.Special perfusion device with multiple chambers enabled 4 groups of brain slices tested simultaneously with 4 different anticonvulsants.Results: 6 cases were studied successfully,brain samples being from 4 epileptic cases(epileptic group)and 2 non-epileptic cases (control group).At lower drug concentrations (μmol/L:phenytoin 100,sodium phenobarbital 1000,diazepam 30,carbamazepine 100) the amplitude of epileptic discharges was less inhibited in epileptic group than in control group.But the inhibition to both groups became similar and remarkable when the drug concentrations were increased 3 folds.Conclusion:Epileptic foci excised from patients could be used in the study of drug sensitivity and the samples from epileptic patients were less sensitive to antioconvulsants than those from non-epileptic patients.
