摘 要:目的探讨磁敏感加权成像(SWI)在脑内微出血(CMBs)影像诊断中的价值。方法对怀疑脑血管病的患者行磁共振(MR)常规头颅平扫,T1加权、T2加权、T2FLAIR、弥散加权及SWI序列扫描。对于常规序列所显示的CMBs数目、敏感性与SWI序列进行比较。结果 231例中共有27例存在脑内微出血,27例中22例有高血压病史,合并出血性脑卒中患者14例,缺血性脑卒中患者7例,混合性卒中患者6例。SWI序列共检出了CMBs灶187个;T1加权图像共检出CMBs灶20个,敏感性11%;T2加权图像共检出CMBs 39个,敏感性11%;T2FLAIR图像共检出CMBs 40个,敏感性22%;弥散加权图像共检出CMBs灶23个,敏感性13%。SWI序列对CMBs的检出率最高。结论 SWI序列对CMBs的检出敏感性要高于常规序列,SWI能明显提高CMBs病灶的检出数目,对脑卒中的治疗与预后评价具有一定的临床参考意义。Objective To investigate the value of susceptibility weighted imaging (SWI) in diagnosis of cerebral mierobleedings (CMBs). Methods The patients who were suspected for cerebral vascular disease were underwent conventional head MR and SWI scanning. The quantity of CMBs and sensitivity in detecting CMBs was compared between the conventional MR images and SWI images. Results In 231 patients, 27 patients had CMBs, and 187 CMBs were detected on SWI images. There were 20, 39, 40 and 23 CMBs on TIWI, T2WI, T2FLAIR and DWI images, and the sensitivity for TIWI, T2WI, T2flair and DWI images in detecting CMBs were 11%, 20%, 22% and 13%, respectively. The quantity of CMBs on SWI images was the largest. Conclusion The quantity of CMBs detected can be obviously increased using SWI images, which has a certain clinical value for the treatment and prognosis of cerebralapoplexy.
