摘 要:目的探讨前列腺外周带癌内分泌治疗不同时间后癌区和非癌区表观扩散系数(ADC)值的变化情况。方法对经穿刺活检证实为前列腺外周带癌患者10例(对照组)以及11例内分泌治疗时间<6个月、14例内分泌治疗时间>6个月的前列腺外周带癌患者行DWI。依病理结果将前列腺外周带6分区归为癌区和非癌区,测量每个分区的ADC值,并对结果进行比较。结果 (1)对照组、内分泌治疗时间<6个月组癌区和非癌区ADC值比较差异有统计学意义(t=-3.872,P<0.05;t=-3.120,P<0.05);内分泌治疗时间>6个月组癌区与非癌区ADC值差异无统计学意义(t=-0.239,P>0.05)。(2)对照组、内分泌治疗时间<6个月组、内分泌治疗时间>6个月组3组癌区ADC值组内比较差异有统计学意义(F=35.430,P<0.05),组间两两比较差异均有统计学意义(P<0.05)。结论内分泌治疗后,前列腺外周带癌癌区和非癌区的ADC值会发生变化;随着治疗时间的延长,癌区和非癌区ADC值的变化趋势不同。Objective To evaluate the changes of the apparent diffusion coefficient values of cancerous and noncancer ous areas of prostatic peripheral zone cancer after different time length of endocrinotherapy. Methods Diffusion weigh ted imaging were performed in 35 patients with diagnosed prostatic peripheral zone cancer, including 10 patients without en docrinotherapy as the control group, 11 patients who were treated with endocrinotherapy less than 6 months, and 14 patients who were treated with endocrinotherapy more than 6 months. According to the pathological results, the peripheral zone was divided into six areas (cancerous and noncancerous aeras). The ADC value of each area was measured, and the resuhs were compared. Results ( 1 ) In control group, the ADC values of the cancerous and noncancerous areas were (0.95± 0. 17) x 103mm2/s and (1.17 ±0.27) x 10mmZ/s. There was statistical significant difference (t = 3. 872, P 〈0.05) between cancerous and noncancerous areas in control group. In group treated with endocrinotherapy less than 6 months, the ADC values of the cancerous and noncancerous areas were ( 1.09 ±0.12) x 10 3mm2/s and ( 1. 16 +0.06) x 10 3mm2/ s. There was statistical significant difference (t = 3. 120, P 〈 0.05 ) of ADC values between cancerous and noncancerous areas in this group. In group treated with endocrinotherapy more than 6 months, the ADC values of the cancerous and non cancerous areas were ( 1.22 ±0.17) x 103mm/s and ( 1.23±0.22) 103mm2/s. There was no statistical significant difference ( t = 0. 239, P 〉 0.05 ) could be detected in this group. (2) For the above mentioned three groups, statisti cal significant difference ( F = 35. 430, P 〈 0.05 ) of the ADC values in the cancerous areas were detected between groups,and statistically significant differences also existed between any two groups (P 〈 0.05 ).
