摘  要:【目的】探讨体外受精-胚胎移植(IVF-ET)过程中胚胎培养液氨基酸浓度与胚胎移植后妊娠结局的关系。【方法】对30例接受IVF-ET治疗且于受精后第5天行胚胎移植的患者,于胚胎移植当日收集使用过的胚胎培养液,高效液相色谱法测定培养液中20种游离氨基酸的浓度,分析其与IVF-ET后妊娠结局的关系。【结果】(1)妊娠组、未妊娠组囊胚培养液谷氨酰胺浓度的中位数分别为60.8和84.7μmol/L,丙氨酸浓度的中位数分别为61.9和92.3μmol/L,两组比较,差异均有统计学意义(P分别为0.029和0.023)。(2)多因素回归分析在校正受精后第3天(D3)细胞数、D3胚胎分级和受精后第5天(D5)胚胎分级的影响后,囊胚培养液中谷氨酰胺和丙氨酸的浓度仍然与妊娠呈负相关(优势比[OR]分别为0.99和0.98,P分别为0.024和0.019)。【结论】囊胚培养液中谷氨酰胺和丙氨酸的浓度与IVF-ET后能否妊娠有关,囊胚培养液中谷氨酰胺和丙氨酸的浓度可能与胚胎活力有关。[Objective] To observe the relationship between the amino acid concentrations of spent culture medium and the outcome of in vitro fertilization (IVF-ET).[Method] A total of 30 patients undergoing in vitro fertilization (IVF) treatment with embryos transferred on day 5 post fertilization were studied.Sixty spent media samples were individually collected after embryo transfer.High performance liquid chromatography (HPLC) was used to analyze the amino acid concentrations of the spent medium from day 3 to day 5 of development.[Results] (1) In pregnancy group,the concentrations of glutamine and alanine were significantly lower than those in non-pregnancy group with the P value of 0.029 and 0.023 respectively.The median concentrations of glutamine were 60.8 and 84.7 μmol/L respectively.The median concentrations of alanine were 61.9 and 92.3 μmol/L respectively.(2) Multivariate logistic regression analysis showed that the concentrations of glutamine (OR =0.99,P =0.024) and alanine (OR =0.98,P =0.019) of blastocyst medium were significantly correlated with a clinical pregnancy,independent of known predictors,such as embryo cell number and embryo morphological grade.[Conclusion] he concentrations of glutamine and alanine in the spent blastocyst culture medium are associated with the pregnancy of IVF-ET,which indicates that the concentrations of glutamine and alanine might be related with the viability of in vitro cultured embryos.
