眼前段生物学结构与青光眼激发试验的关系

摘　　要：【目的】探讨房角狭窄者眼前段结构参数与青光眼暗室激发试验阳性的相关关系。【方法】回顾性分析260例房角狭窄者的眼前段生物学参数和青光眼暗室激发试验结果。所有患者应用超声生物显微镜定量测量眼前段生物学参数,包括前房深度(ACD)、瞳孔直径(PD)、晶状体矢高(LV)、周边虹膜厚度(IT)、虹膜膨隆度(IC)、房角开放距离(AOD)、小梁网-睫状突夹角(TCPA)和房角同位关闭的象限数(NPAC);所有患者均完成青光眼暗室激发试验。比较暗室激发试验阳性者与阴者间的眼前段结构差异,分析青光眼暗室激发试验中眼压升高的相关因素。【结果】房角狭窄者青光眼暗室激发试验的阳性率为27.3%;暗室试验中眼压升高的幅度与AOD(r=-0.249,P<0.001)、IC(r=-0.125,P=0.043)负相关,与IT(r=0.188,P=0.002)、NPAC(r=0.305,P<0.001)正相关;暗室试验阳性者的IT和NPAC大于阴性者,AOD小于阴性者。【结论】房角和虹膜参数与青光眼暗室激发试验结果相关,房角狭窄、房角同位关闭、厚虹膜是暗室激发试验阳性的危险因素。[Objective] To investigate the relationship of positive dark-room provocative test and anterior segment parameters measured by ultrasound biomicroscopy （UBM） in the patients with narrow anterior chamber angle.[Methods] Dark-room provocative test and anterior segment parameters measured by UBM were analyzed retrospectively in two hundred and sixty cases with narrow anterior chamber angle.Anterior chamber depth （ACD）,pupil diameter （PD）,lens vault （LV）,peripheral iris thickness （IT）,iris convex （IC）,anterior chamber angle open distance （AOD）,trabecular-meshwork ciliary process angle （TCPA）,number of positional angle closure （NPAC） were quantitatively analyzed.All cases underwent provocative test.Difference of anterior parametres between eyes with positive and negative provocative test and parametres correlated with IOP elevation were analyzed.[Results] Seventy-one out of 260 patients had IOP elevation more than 8 mmHg at least in one eye.The total positive rate were 27.3%.IOP elevation in dark room provocative test was negatively correlated with AOD （r =-0.249,P 〈 0.001） and IC （r =-0.125,P =0.043）,and positively correlated with IT （r =0.188,P =0.002） and NPAC （r =0.305,P 〈 0.001）.IT（449.91 vs 422.36 μm; P 〈 0.001） and NPAC （1.59 vs 0.62; P 〈 0.001） were greater and AOD （60.24° vs 98.7° ; P 〈 0.001） was smaller in patients with positive results than those with negative results.[Conclusions] Parameters of anterior chamber angle and iris were related with positive dark room test.Narrower chamber angle,appositional angle closure and thicker iris were risk factors for positive results in dark room provocative test.
