慢性阻塞性肺疾病和支气管哮喘患者中ADAM33基因功能多态性及与吸烟的交互作用

摘　　要：目的探讨ADAM33基因外显子多态及其与吸烟的交互效应,并探讨其与我国人群慢性阻塞性肺疾病(COPD)和支气管哮喘(简称哮喘)发病的关系。方法应用病例对照研究方法,采用Taqman技术检测300例COPD患者、220例哮喘患者和320例正常对照者6个常见外显子多态(rs3918392A>G、rs2271511C>T、rs3918396A>G、rs528557G>C、rs2280091A>G、rs2280090A>G)的基因型,应用logistic模型分析各多态位点及其与吸烟的交互效应,并分析其与COPD和哮喘发病风险的关系。结果 rs2280091A>G和rs2280090A>G与COPD和哮喘发病皆有显著关联(P<0.05)。合并两位点后,危险基因型个数与吸烟在增加COPD风险上有显著的交互效应(P=0.016),该交互效应在哮喘风险上接近统计学显著性水平(P=0.060)。此外,未发现其余4个基因多态性与COPD和哮喘发病有显著关联。结论 ADAM33基因外显子多态rs2280091A>G和rs2280090A>G能增加我国人群COPD和哮喘的发病风险,是COPD和哮喘的共有遗传易感位点。Objective To test the roles of the interaction between polymorphisms in the exons of ADAM33 and smoking in the risks of chronic obstructive pulmonary disease （ COPD ） and asthma in Chinese. Methods In a case - control study, we genotyped 6 common exon polymorphisms of ADAM33 by the Taqman assay in a total of 300 COPD ca- ses, 220 asthma cases and 330 normal controls. Their associations with risks of COPD and asthma, and their interactions with smoking were analyzed with logistical regression. Results The rs2280091A 〉 G and rs2280090A 〉 G were both sig- nificantly associated with risks of COPD and lung cancer （P 〈 0. 05 ） ; moreover, the risk genotypes significantly interacted with smoking on COPD risk （P = 0. 016） , while this interaction effect was on the borderline significance on asthma risk （P = 0. 060）. However, no significant correlation was observed between the other 3 polymorphisms and either the risk of COPD or lung cancer. Conclusion The two polymorphisms （rs2280091A 〉 G and rs2280090A 〉 G） exerts significant risk of COPD and asthma in Chinese, suggesting the common genetic susceptible loci for COPD and asthma.
