奥美拉唑、兰索拉唑和泮托拉唑预防重症脑卒中患者应激性溃疡的成本—效果分析

摘　　要：目的探讨3种质子泵抑制剂预防重症脑卒中患者应激性溃疡的成本—效果。方法 310例重症脑卒中患者随机分为3组;A组105例,B组102例,C组103例。3组均予常规治疗,A组采用奥美拉唑预防,B组采用兰索拉唑预防,C组采用泮托拉唑预防。比较各组的溃疡发生率、医院获得性肺炎发病率和总病死率,并进行成本—效果分析。结果 3组溃疡发生率、预防应激性溃疡出血的有效率比较差异无统计学意义(P>0.05)。与B组和C组比较,A组医院获得性肺炎发病率较高,差异有统计学意义(P<0.05);A组、C组病死率比较差异有统计学意义(P<0.05),B组和C组比较差异无统计学意义(P>0.05)。3组成本—效果分别为23.5,23.1和11.4。结论泮托拉唑成本效果优于奥美拉唑和兰索拉唑。Objective To investigate the cost-effectiveness of cmeprazole, lansoprazole and pantoprazole in the pre- vention of stress ulcer in patients with severe cerebral stroke. Methods 310 cases of patients with Severe stroke were random- ly divided into 3 groups as group A （ 105 cases） ,group B （ 102 cases） and group C （ 103 cases）. The 3 groups were given conventional therapy, the group A was prevented with omeprazole, the group B was prevented with lansoprazole, the group C was prevented with pantoprazole. The incidence of ulcer, the incidence of hospital acquired pneumonia and the overall mortality rate were observed and compared in the 3 groups. And do cost effectiveness analysis. Results There were significant differences in aspect of incidence of ulcer, efficiency rate of stress ulcer bleeding prevention in the 3 groups （ P 〈 0.05 ）. The incidence of hospital acquired pneumonia of group A was higher than that of group B and group C,and the difference was statistically signifi- cant （ P 〈 0.05）. There was significant difference in aspect of mortality rate between group A and group C （ P 〈 0. 05 ）, but there was no significant between group B and group C （P 〉 0.05）. The cost-effectiveness of was respectively 23.5,23.1 and 11.4. Conclusions The cost-effectiveness of pantoprazole is better than that of omeprazole and lansoprazole.
