鼻咽癌临床靶区及危及器官计划体积的确定  

Planning Volume's Determination for Clinical Target Regions and Organs at Risk Before Radiotherapy of Nasopharyngeal Carcinoma

摘　　要：目的:通过对鼻咽癌患者的摆位误差分析,确定鼻咽癌患者临床靶区(CTV)外扩计划靶区(PTV)边界值的大小及外扩危及器官的计划体积(PRV)的边界值大小。方法:对2013年5-6月份的14例鼻咽癌患者的摆位误差进行测定分析。患者采用仰卧位、头颈肩热塑体膜固定技术,摆位标记标于热塑体膜上。患者初次治疗前和以后每周均需通过千伏级锥形束CT(KV CBCT)对患者治疗位置采集CT影像(层厚3mm),并与治疗计划采用的CT影像进行比对,医生认可比对结果后,记录下各方向摆位误差值(旋转误差没有考虑)。临床靶区(CTV)至计划靶区(PTV)的边界值的由经验公式MPTV=2.5Σ+0.7σ计算得出;危及器官外扩PRV的边界值由公式MPRV=1.3Σ+0.5σ计算得出(Σ为系统误差标准差,σ为随机误差标均方根)。结果:患者在x、y、z轴(x、y、z分别表示患者左右、头脚和前后方向)误差(系统误差±随机误差)分别为(-0.7±1.40)、(-0.39±1.66)和(-0.14±1.21)mm。临床靶区外扩计划靶区的边界值在左右、头脚和前后方向依次为2.38,2.64和3.17mm;危及器官外扩PRV的边界值在左右、头脚和前后方向依次为1.43,1.60和1.81mm。结论:建议在进行鼻咽癌放射治疗时外扩PTV边界值依次为左右方向3mm、头脚方向3mm、前后方向3.5mm;危及器官(视神经、视交叉、脑干)外扩PRV边界值依次为左右方向1.5mm、头脚方向2mm、前后方向2mm;脊髓的PRV边界值为各方向6mm。Objective： To analyze the set-up errors in radiotherapy for nasopharyngeal carcinoma （NPC） and to determine the margins required for clinical target volume （CTVs） and organs at risk vol- ume （OARs） respectively to plan target volume （PTVs） and organs at risk volume （PRVs）. Methods： The set up errors of 14 patients in radiotherapy for NPC during May to June in 2013 were measured and analyzed in this study. Linear errors （x, y, z axis） were obtained by cone beam CT （CBCT）. MpTv and MpRvwere determined. Results.. Fourteen patients received a total of 84 CBCT scans before treatment. The systematic errors on z（left-right）, y（superior-inferior）, and z （anterior-posterior） directions were（-0.7±4-1.40） ram, （-0.39±1.66） mm, and （-0.14 ±1.21） mmrespectively. Mpv was 2. 38 mm in x axis, 2.64mminyaxis, and 3.17mminzax-is. MeRvWaS 1. 43 mm in x axis, 1. 60 mm in y axis, and 1.81 mm in z axis. Conclusion： Set-up errors were unavoidable in NPC irradiation. To minimize the influence of set-up errors, we sug- gest a PTV margin of 2.5 ram, 3 mm and 3.5 mm on x, y, z directions, respectively and a PRV margin of 1.5 ram, 2 ram, 2 mm on x, y, z directions, respectively for optic nerve, optic chiasm and brain stem and a PRV margin of 6 mm for spinal cord.
