摘  要:目的探讨颅内软骨瘤的CT、MRI表现。方法回顾性分析10例经病理证实的颅内软骨瘤的CT、MRI表现;行CT平扫9例,增强2例,MR平扫10例,增强9例。结果9例位于颅底并突人颅内生长,1例位于额顶部大脑镰旁,均伴占位效应并推压邻近脑组织。CT上9例病灶呈低密度或等密度伴不同程度的钙化,边缘均清楚,其中3例病灶周缘见不完整高密度环形、弧形包膜,相邻骨质4例吸收破坏、1例反应性增生。10例MRI病灶均呈混杂信号,其中7例呈以稍长T1稍长T2为主的混杂信号,3例呈以长T1、T2为主的混杂信号,其内钙化低信号。增强后,CT上2例病灶呈斑片状轻度或中度强化,MR上4例肿块呈明显边缘环状强化、内部分隔状及小结节状、小环状强化,形似蜂窝,5例呈斑片状欠均匀明显或中度强化。结论颅内软骨瘤的CT及MRI影像学表现具有一定特征性,结合临床并综合其影像学表现,可提高其诊断准确率,最终确诊依赖于病理。Objective To investigate the CT and MRI features of intracranial chondroma. Methods CT and MRI findings of 10 cases with intracranial chondroma confirmed by pathology were retrospectively analyzed. The plain CT images of 9 cases and en hanced CT images of 2 cases were obtained. And the conventional MRI images of all 10 cases and enhanced MRI images of 9 cases were also obtained. Results 9 cases of intracranial chondroma were located at the skull base and project into brain, 1 case was loca ted at the region of cerebral falx, all cases were associated with mass effect and extruding the adjacent brain tissue. On CT images, the lesions showed hypodense or isodense area with calcification in varying degrees and clear border in 9 cases, 3 cases of which with partial circular high density edge and embowed capsul,and with adjacent bone invasion in 4 cases, with adjacent bone reactive hyper plasia in 1 case. On MRI, all cases showed mixed intensity signal, the tumors presented heterogeneous slightly hypo-intense on T1WI images, and slightly hyper-intense on T2WI images in 7 cases. 2 cases showed slightly or moderately enhancement on CT scans. On MRI scan, 4 cases showed intense ring enhancement on the edge, with separate, nodular, small ring enhancement in the inside, which demonstrated typical "honeycomb" sign ,and 5 cases appeared patchy mild or obvious enhancement. Conclusion Intracranial chondroma,has some characteristic CT and MRI features ,and its diagnostic accurate rate can be improved in combination both the CT and MRI features and the patient's clinical information. The final diagnosis still depends on pathology.
