摘　　要：目的评价人乳头瘤病毒（HPV）L1壳蛋白阴性表达诊断宫颈上皮内瘤变（CINⅡ）及以上宫颈病变的价值。方法应用诊断性试验的Meta分析思路检索国内外相关文献，以合并的敏感性、特异性、阳性似然比、阴性似然比、比值比和95％可信区间（95％CI），ROC曲线下面积（AUC）和Q值等为合并效应量对HPVLl壳蛋白阴性表达诊断CINⅡ及以上宫颈病变的价值进行拟合ROC曲线分析。结果纳入研究的12篇文献HPVL1壳蛋白阴性表达诊断CINII及以上宫颈病变的合并敏感性为0．81（95％CI：0．78～0．83）、特异性为0．50（95％CI：0．47-0．53）、阳性似然比为1．75（95％CI：1．41—2．18）、阴性似然比为0．38（95％CI：0．29—0．51）、比值比为4．75（95％CI：2．92-7．72）、AUC为0．74、Q值为O．68。结论HPVL1壳蛋白阴性表达用于CINⅡ及以上宫颈病变诊断的准确性中等，敏感性较高，但特异性较低，仅适用于CINII及以上宫颈病变的筛查。Objective To assess the diagnostic value of negative expression of HPV L1 capsid protein for cervical intraepithelial neoplasia （CIN） U and above. Methods According to meta-analysis of diagnostic test method, eligible literatures were searched and fitting ROC curve was performed to assess the diagnostic value of negative expression of HPV L1 capsid protein. The results were expressed as pooled sensitivity, specificity,positive likelihood ratio （＋LR）, negative likelihood ratio （-LR）, diagnostic odds ratio （DOR） and 95% corffidence intervals （95%C/）, area under curve （A UC） and Q value.Results There were 12 eligible literatures included in this analysis. Fitting ROC curve analysis showed the pooled sensitivity,specificity,＋LR ,-LR, DOR, A UC and Q value were 0.81 （95%CI：0.78-0.83）, 0.50 （95%CI：0.47-0.53）, 1.75 （95%CI：1.41-2.18）, 0.38 （95%C1：0.29-0.51）, 4.75 （95%CI：2.92- 7.72）, 0.74 and 0.68, respectively. Conclusions The accuracy of diagnosis is medium according to HPV L1 capsid protein negative expression for the diagnostic of patients with CIN 11 and above, and the sensitivity is high, but the specificity is low, so HPV L1 capsid protein may useful for the screening of patients with CIN I1 and above.
