摘  要:目的 :寻找一种快速有效的临床诊断单疱角膜炎 (HSK)的客观依据。方法 :应用多聚酶链反应 (PCR)技术 ,分别以多聚酶基因、胸腺嘧啶激酶基因和糖蛋白 D基因引物 ,检测正常人和各种角膜炎病人的泪液及角膜上皮中单纯疱疹病毒 (HSV) DNA。结果 :正常人全部为阴性 ,树枝状和地图状角膜炎 2 6例中 2 4例阳性 ;盘状角膜炎 16例中 14例为阴性 ;浅层点状角膜炎 32例中19例阳性 ,其他未定性角膜炎 33例中 14例阳性。同时对 HSK病变过程进行监测 ,显示在发病 2~ 15天 HSV DNA的检出阳性率最高 ;发病 40天或用药 30天左右转阴。并对泪液和角膜上皮不同取材方法进行比较 ,二者间无显著差异 ,可优选泪液。结论 :PCR可以作为To find a rapid,effective and objective basis for clinical diagnosis of herpes simplex keratitis(HSK).Methods:By polymerase chain reaction(PCR),we detected Herpes Simplex Virus(HSV)DNA in tears and cornea epithelials of keratitis patients and control.Polymerase gene primer,thymidylate kinase gene primer and glucoprotein D gene primer were used respectively.Results:Control were negative.24 of 26 dendritic and mapping keratits were HSV DNA positive.14 of 16 disk keratitis were negative.19 of 32 superficial punctate keratitis were positive.And 14 of 33 others were positive including those can not be diagnosed It suggests at the same time that PCR positive rate within 2 15 days after infection is the highest.And after 40 days of infection or 30 days of treatment,PCR of most samples turned to negative.Different samples including tears and cornea tissues were contrasted.and no obvious differences were found.Thus we can say tears are the first choice sample.for they are easy to obtain.Conlusion:PCR can be a pathogenic diagnostic method of HSK.
