图像引导下大分割放疗对局部晚期非小细胞肺癌患者血清肿瘤标志物及转移侵袭相关指标的影响研究

摘　　要：目的 研究探讨图像引导下大分割放疗对局部晚期非小细胞肺癌患者血清肿瘤标志物及转移侵袭相关指标的影响.方法 选取2010年4月～2013年1月于河北医科大学第三医院进行治疗的64例局部晚期非小细胞肺癌患者为研究对象,将其随机分为对照组和观察组每组各32例,对照组以常规分割三维适形放疗进行干预,观察组则以图像引导下大分割放疗进行干预,然后将两组不同分期者的总有效率及治疗前、治疗后6、12周的血清肿瘤标志物、转移侵袭相关指标进行比较.结果 观察组Ⅲa期和Ⅲb期的总有效率分别为76.47％和66.67％,均高于对照组（x2=6.542、6.831,P＜0.05）,治疗后6、12周的血清者肿瘤标志物则低于对照组,而转移侵袭相关指标变化也大于对照组,观察组治疗后6周与12周均优于治疗前,差异有统计学意义（均P＜ 0.05）.结论 图像引导下大分割放疗对局部晚期非小细胞肺癌患者的治疗效果较好,且对于血清肿瘤标志物及转移侵袭相关指标的影响也较大.Objective To study and investigate the influence of hypofractionated radiotherapy guided by image for serum tumor markers,invasion and metastasis related indexes of patients with local advanced non-small cell lung cancer.Methods 64patients with local advanced non-small cell lung cancer from April 2010 to January 2013 in the Third Hospital of Hebei Medical University were selected as research object,and they were randomly divided into control group and observation group with 32 cases in each group.The control group was intervened with conventional segmentation of three-dimensional conformal radiotherapy,the observation group was intervened with hypofractionated radiotherapy guided by image,then the total effective rate of different stages,serum tumor markers,invasion and metastasis related indexes before the treatment and at the 6th and 12th week after treatment of two groups were compared.Results The total effective rate of observation group with stage Ⅲa and Ⅲb were 76.47％ and 66.67％,and they were higher than those of control group （x2=6.542,6.831,P ＜ 0.05）,serum tumor markers at the 6th and 12th week after the treatment were lower than those of control group,change of invasion and metastasis related indexes were bigger than those of control group,and those indexes of observation group at the 6th and 12th week after the treatment were all better than those before the treatment,there were all significant differences （all P ＜ 0.05）.Conclusion The effect of hypofractionated radiotherapy guided by image in the patients with local advanced non-small cell lung cancer is better,and its influence for the serum tumor markers,invasion and metastasis related indexes is great.
