摘　　要：目的 评价吉西他滨联合奈达铂诱导化疗后放疗治疗中晚期鼻咽癌的疗效、毒副反应及后遗症,探讨局部中晚期鼻咽癌治疗的更好组合模式.方法 中晚期鼻咽癌98例采用吉西他滨联合奈达铂诱导化疗后,同期奈达铂化疗调强放疗治疗.吉西他滨1 000mg/m2,d1、d8,静脉点滴;奈达铂20mg/m2,d1~3;3周方案,共两个疗程.放疗为诱导化疗后的靶区,行容积调强放疗（V-MAT）,放疗同期每周奈达铂化疗,奈达铂30mg/周.评价其毒副反应、近期疗效、远期疗效及后遗症.结果 98例患者均完成这一方式的治疗,近期有效率及缓解率100%.毒副反应以胃肠道反应及血液系统毒性较为多见,常规处理可恢复.结论 吉西他滨联合奈达铂诱导化疗后放疗同期治疗中晚期鼻咽癌疗效明显、副作用低.Objective To evaluate the curative effect and adverse reaction of radiotherapy after gemcitabine combined nedaplatin inductive chemotherapy treatment for local advanced nasopharyngeal carcinoma,to explore the better combination mode of treatment of local advanced nasopharyngeal carcinoma （NPC）. Methods 98 cases of nasopharyngeal carcinoma （NPC） were treated by intensity modulated radiation therapy after gemcitabine combined nedaplatin inductive chemotherapy pe- riod. Patients accepted gemeitabine 1 000mg/m^2, d1, 8,intravenous drip; nedaplatin 20mg/m^2, d1-3; three weeks as a course, a total of two course of treatment. Radiation inductive chemotherapy after the target intensity modulated radiation therapy （IMRT） line volume （V-MAT）, nedaplatin,radiation at the same time each week platinum chemotherapy, nedaplatin 30 nag/week. Evalu- ated its recent curative effect, adverse reaction and the forward curative effect and complications. Results All patients com- pleted the treatment and the rate of recent efficient and response was 100%. Adverse reaction mainly was the gastrointestinal tract and blood system toxicity and restored after conventional treatment. Conclusion Gemcitabine combined nedaplatin in- ductive chemotherapy during the same period treatment of the advanced nasopharyngeal carcinoma （NPC） has obvious curative effect and side effects is low.
