电视胸腔镜手术治疗老年非小细胞肺癌的临床观察

摘　　要：目的探讨电视胸腔镜手术对老年非小细胞肺癌患者的有效性和安全性。方法选取本院54例老年非小细胞肺癌患者,随机均分为电视胸腔镜组(全胸腔镜下肺癌根治术)和常规开胸组(传统开胸肺癌根治术),比较两组患者的手术时间、切口长度、术中出血量、引流管留置时间、术后平均住院时间、治疗总有效率和并发症发生率的差异。结果所有患者术后病情均得到不同程度的改善,康复出院,无一例死亡。电视胸腔镜组患者的手术时间、切口长度、术中出血量、引流管留置时间、术后平均住院时间和并发症发生率均显著少于常规开胸组,差异有统计学意义(P<0.05);两组患者的治疗总有效率比较差异无统计学意义(P>0.05)。结论采用电视胸腔镜手术治疗老年非小细胞肺癌患者安全有效,值得临床推广应用。Objective To investigate the clinical efficacy and safety of video-assisted thoraeoscopic surgery （VATS） in the treatment of elderly patients with non-small cell lung cancer （NSCLC）.Methods 54 cases of elderly patients with NSCLC in our hospital were randomly divided into VATS group （received full thoraeoseopie lung resection） and con- ventional thoracotomy group （received conventional thoracotomy lung reseetion）.The operative time,incision length, blood loss,drainage tube indwelling time,mean postoperative hospital stay,total effective rate and complication rate of two groups was compared.Results The operative time,incision length,blood loss,drainage tube indwelling time,mean postoperative hospital stay and complication rate of VATS group was significantly less than that of conventional thora- eotomy group respectively （P〈0.05）;The total effective rate of two groups had no significant difference （P〉0.05）.Con- dusion VATS is safe and feasible for the elderly patients with NSCLC.It＇s worthy of clinical widely used.
