再程放化疗在食管癌放疗后复发患者中的应用分析

摘　　要：目的分析食管癌放疗后复发患者应用再程放化疗治疗的价值。方法选择2010年11月至2012年12月符合纳入标准的食管癌放疗后复发患者97例,随机分为治疗组(三维适形放-化疗联合)48例和对照组(单纯三维适形放疗组)49例。比较两组患者近期缓解率、前3年生存率和不良反应。结果治疗组与对照组治疗结束后,患者近期缓解率分别为77.3%(34/44)和80.4%(37/46),差异无统计学意义(χ2=0.135,P=0.713);治疗组第1、2、3年生存率略高于对照组,但差异无统计学意义(P>0.05);两组患者不良反应发生情况比较,治疗组明显高于对照组,两组比较差异有统计学意义(P<0.05)。结论对于食管癌放疗后复发患者应用再程放化疗治疗是有效的治疗手段,但是相比单纯三维适形放疗,其毒副作用明显增加,因此应慎重选择。Objective To investigate the value of radiotherapy concurrent with chemotherapy in recurrence from esophageal carcinoma af- ter radiotherapy. Methods Total 97 cases of patients with recurrence from esophageal carcinoma after radiotherapy from Nov 2010 to Dec 2012 were randomly divided into treatment group （ three dimensional conformal radiotherapy concurrent with chemotherapy, 48 cases） and control group （radiotherapy, 49 cases）. The response rate, first 3 year survival rate and adverse reaction occurrence rate were compared between those two groups. Results The response rate was 77.3% （ 34/44 ） in treatment group and 80.4% （ 37/46 ） in control group. There was not significant difference （X2 =0. 135, P =0. 713）. The 1,2,3 year survival rate were slightly higher in treatment group than those in control group. However, the difference was not significant （ P 〉 0.05）. The adverse reaction occurrence was significantly higher in treatment group compared with control group （ P 〈 0. 05 ）. Conclusion Radiotherapy eoncurrent with chemotherapy is an effective therapy in recurrence from esophageal carcinoma af- ter radiotherapy. However, it should be applied after careful consideration for its more obvious adverse reactions compared with radiotherapy.
