小切口非超声乳化人工晶状体植入术治疗528例老年性白内障

摘　　要：目的研究小切口非超声乳化人工晶状体植入术治疗老年性白内障的临床疗效及安全性。方法回顾性分析淮北市人民医院眼科自2010年4月至2013年4月收治的528例620眼行小切口非超声乳化人工晶状体植入术治疗的老年性白内障患者的临床资料。观察手术并发症及术后视力恢复情况,同期入院行超声乳化白内障吸出术的200例240眼老年患者为对照组。结果术后第1天裸眼视力≥0.1者598眼,脱盲率为96.5%;≥0.3者568眼,脱残率为91.6%。术后1周随访,裸眼视力≥0.1者610眼,脱盲率为98.4%。≥0.3者588眼,脱残率为94.8%;人工晶体植入615眼,置入率为99.2%。术中及术后少数老年患者出现不同程度的并发症,轻度虹膜炎症25眼(4.0%),角膜内皮轻度水肿34眼(5.5%),后囊破裂8眼(1.3%),一过性高眼压6眼(1.0%),透明皮质残留6眼(1.0%),对症处理后很快得到治愈,无其他严重并发症。结论采用小切口非超声乳化术治疗白内障效果良好,安全性高,并发症少,手术效果满意,适合我国基层白内障复明工程。Objective To study the clinical effect and safety of small incision phaeoemulsifieation and intraoeular lens im- plantation in the treatment of senile cataract. Methods From April 2010 to April 2013, the clinical data of 528 cases （620 eyes） of small incision phaeoemulsifieation and intraoeular lens implantation in the treatment of senile cataract patients were retrospectively analyzed. Surgical complications and postoperative visual acuity were observed. In the same period, 200 cases （ 240 eyes） of elderly patients treated by phacoemulsification surgery were ehoosed as control group. Results On the first clay after surgery, the uncorrected visual acuity ~〉 0. 1 in 598 eyes, literacy rate was 96. 5% ; 1〉 0. 3 in＇568 eyes, residue removal rate was 91.6%. On the postoperative week follow-up, visual acuity t〉 0. I in 610 eyes, literacy rate was 98.4% ; ~〉 0. 3 in 588 eyes, residue removal rate was 94. 8%. Intraoeular lens implantation in 615 eyes, the placement rate was 99.2%. During surgery and postoperatively, small number of elderly patients had varying degrees of complications, mild inflammation of the iris in 25 eyes （4.0%） , the corneal endothelial mild edema in 34 eyes （5.5%） , posterior capsular rupture in 8 eyes （ 1.3% ） , transient high intraoeular pressure in 6 eyes （0. 97% ）, transparent cortex residues in 6 eyes （0. 97% ） , symptomatic treatment to be cured soon after, no other serious complications. Conclusion The effect of small incision non-phacoemulsifieation cataract is good, it is safe, has few complications, satisfactory surgical results, so it is suitable to Chinese grassroots cataract project.
