摘  要:目的探讨增殖细胞核抗原(PCNA)表达在甲状腺肿瘤病理学诊断和鉴别诊断中的意义及应用价值。方法采用 免疫组化LSAB法检测14例甲状腺腺瘤,10例不典型腺瘤,15例乳头状腺癌,10例滤泡性腺癌,10例髓样癌,10例未 分化癌。结果 PCNA增殖指数在甲状腺腺瘤为 12.8,不典型腺瘤为 19.8,乳头状癌为 19.2,滤泡性腺癌为 26.4,髓样癌 为27.9,未分化癌为44.3。各组间比较:不典型腺瘤与乳头状癌之间无差异;腺瘤与不典型腺瘤和乳头状癌间有显著性 差异(P<0.05),与其余各组有极显著性差异(P<0.01)。结论PCNA增殖指数可作为判断甲状腺肿瘤良恶性的指标,并 可反映其临床生物学行为,为预后提供重要信息。Objective To evaluate the significance and practical value of examining the expression of proliferating cell nuclear antigen (PCNA) in the differentiation and diagnosis of thyroid tumors. Methods The expression of PCNA in the tumor tissues in 14 cases of thyroid adenoma, 10 cases of atypical adenoma,15 papillary carcinoma, 10 follicular carcinoma, 10 medullary thyroid carcinoma and 10 undifferentiated carcinoma were determined by immunohistochemical method with strept avidin-biotin (LSAB). Results PCNA proliferating index were 12.8 in thyroid adenoma,19.8 in atypical adenoma, 19.2 in papillary carcinoma, 26.4 follicular carcinoma, 27.9 in medullary thyroid carcinoma and 44.3 in undifferentiated carcinoma. No significant difference in the PCNA proliferating index between thyroid adenoma and papillary carcinoma was observed,while that between thyroid adenoma and atypical adenoid tumors along with various carcinomas was found to be significantly different (p<0.01). Conclusions PCNA proliferating index may be useful indicators for the differentiation and diagnosis of thyroid tumors and for the prognostic assessment.
