Ⅲ期卵巢上皮癌首次手术治疗彻底性对预后的影响

摘　　要：目的：探讨Ⅲ期卵巢上皮癌首次手术治疗的彻底性对预后的影响。方法：对216例Ⅲ期卵巢上皮癌患者，行首次肿瘤细胞减灭术，根据术后肿瘤残留灶大小及是否首次手术分成三组：A组125例，残留癌灶＜2 cm；B组91例，残留癌灶≥2 cm；C组18例为外院首次手术加化疗后，我院行肿瘤细胞减灭术，残留癌灶＜2 cm。术后三组进行联合化疗基本相同。结果：A组5年生存率为49.6%，B组5年生存率为8.8%，C组5年生存率为16.7%，B组和C组分别与A组比较差异均有显著性(Ｐ＜0.005和Ｐ＜0.05)。B组与C组比较差异无显著性(Ｐ＞0.05)。结论：Ⅲ期卵巢上皮癌首次手术是治疗关键，残留癌直径＜2 cm时可明显提高患者生存率。Objective:To investigate the effect of prima ry operation completeness of stage Ⅲ ovarian epithelial carcinoma on its progno sis.Methods:Two hundred and sixteen patients with stage Ⅲ ovarian epith elial carcinoma were operated to reduce the tumour cells primarily.The patients were divided into two groups according to the residual tumor's size:group A(n=125),the diameter of the cancer residue＜2 cm and group B(n=91),the ca ncer residue≥2 cm.The two groups were all given chemotherapy postoperatively.Results:The five－year survival rate of group A and group B is 50% a nd 8.6% respectively,the difference of the two groups is significant(Ｐ＜0.0 01). Conclusions:The completeness of primary operation of stage Ⅲ ovaria n epithelial carcinoma is of paramount importance and the diameter of the residu al tumor＜2 cm may improve the patient's survival rate markedly.
