晚期前列腺癌两种去势术对患者的心理影响

摘　　要：目的通过两种睾丸切除术治疗晚期前列腺癌观察患者对不同术式的接受程度以及心理影响,从而选择一种疗效相同患者乐意接受的手术方式。方法选择2010-03—2012-02笔者所在科收治的晚期前列腺癌患者49例,分为睾丸切除组(A组)和白膜下睾丸切除组(B组);术前采用ZUNG焦虑自评量表(Self-rating Anxiety Scale,SAS)进行焦虑程度评估,并对结果进行分析,同时观察术后恢复情况。结果 A组SAS评分明显高于B组(P<0.05,t=2.123);术后精神症状的发生率A组也明显高于B组(P<0.05)。结论白膜下睾丸切除术对于临床晚期前列腺癌特别是高龄患者,在减轻患者术前术后心理焦虑、精神症状以及提高患者的治疗配合度等方面均明显优于睾丸切除,是较理想的手术去势方式。Objective Through two testicular excision in the treatment of advanced prostate cancer to observe pa- tient＇ s acceptance on different procedures and then suffered from psychological influences, so as to choose a procedure with same efficacies and being happy accepted by the patients. Methods From March 2010 to February 2012 in author＇ s de- partment 49 patients with advanced prostate cancer were selected and divided into two groups： testis resection group （group A） and under tunica albuginea testis resection group （group B ） ; preoperative ZUNG＇ s Self - rating Anxiety Scale （SAS） was used for assessing patient＇ s anxiety levels, and the obtained results were analyzed, at the same time the postoperative recovery was observed. Results In group A, SAS score was significantly higher in group B, between two groups there was a significant difference（ P 〈 0. 05, t = 2. 123 ） ; postoperative mental symptom occurrence rate of group A was significantly higher than that of group B （ P 〈 0. 05 ）. Conclusion Under tuniea albuqinea orchidectomy for clinical prostate cancer, elderly patients in particular, in relief of preoperative and postoperative psychological anxiety, psychotic symptoms of pa- tients and improvement of the treatment coordinate of patients is superior to orchiectomy, thus it is an ideal operationally castrated way.
