tPSA,f/tPSA,PSAD在前列腺增生与前列腺癌诊断中的意义

摘　　要：目的：探讨血清总前列腺特异性抗原（tPSA）、游离前列腺特异性抗原（fPSA）与tPSA的比值（f/tPSA）、前列腺特异性抗原密度（PSAD）三项指标在前列腺增生（BPH）与前列腺癌（PCa）诊断与鉴别诊断中的意义.方法：采用化学发光全自动分析仪检测临床诊断且病理确诊为BPH（80例）和PCa（30例）患者血清中tPSA及fPSA并计算f/tP-SA,经直肠彩色超声测定患者前列腺三维尺寸并计算PSAD值,采用SPSS17.0统计软件对所有数据进行统计学分析.结果：BPH与PCa发病主要集中于老年男性,其中BPH发病高峰年龄段为60～80岁.BPH与PCa患者的tPSA,f/tP-SA,PSAD三项指标比较差异均有统计学意义,BPH患者tPSA值多集中在于4～ 10 ng/mL间,差异有统计学意义（P＜0.05）,PCa患者tPSA值均大于4 ng/mL.结论：tPSA,f/tPSA,PSAD三项指标均为BPH及PCa的鉴别指标,应进行老年男性tPSA筛查并进行前列腺健康教育,提高公众认知度,以提高老年男性生活质量.Objective ： To investigate the diagnostic significance of serum total prostate specific antigen （ tPSA）, free pros- tate specific antigen （fPSA） and tPSA （f/tPSA）, prostate specific antigen density （PSAD） for benign prostatic hyperplasia （BPH） and prostate cancer （PCa）. Methods：Eighty patients with BPH and 30 patients with PCa was diagnosised by chemilu- minescence automatic analyzer and pathology, tPSA and fPSA was recorded, f/tPSA was calculated. To calculated PSAD ac- cording to transrectal color ultrasound determination of three dimensions, use SPSS17.0 statistical software for statistical analy- sis of all data. Results ： BPH and PCa incidence mainly in older men, including BPH peak incidence ages 60 ~ 80 years old. tP- SA,f/tPSA,PSAD of patients with BPH and PCa had significant differences,tPSA values are more concentrated in the 4 -10 ng/mL, the difference was statistically significant （P 〈 0.05 ） , tPSA in patients with PCa were higher than 4 ng/mL. Conclu- sion ： Three indexes of tPSA, f/tPSA, PSAD are differential index of BPH and PCa, should be tPSA screening of elderly men and prostate health education, raising public awareness, to improve the quality of life of elderly men.
