摘　　要：目的 探讨中央区乳腺癌切除乳头乳晕复合体的保乳治疗的临床疗效.方法 2002年10月-2012年10月,对43例Ⅰ-Ⅱ期原发性中央区乳腺癌,行癌灶局部扩大切除并腋窝淋巴结清扫的保乳手术,同时切除乳头乳晕复合体.术后常规全乳放射治疗,并根据具体病理及免疫组织化学分析结果,接受化学治疗、内分泌治疗及靶向治疗.结果术后随访7-96个月（中位数38个月）,均无局部复发和远处转移,患侧乳房外形总体优良率86.0%（37/43）（优23例,良14例）.结论 在严格掌握手术指征的前提下,对Ⅰ-Ⅱ期原发性中央区乳腺癌实施切除乳头乳晕复合体的保乳治疗,近期疗效满意,远期效果有待长期随访观察.Objective To observe the clinical effect of breast conservation therapy of resection of the nipple-areola complex in women with early primary central breast cancer. Methods Forty-three patients with stage I to stage ]I primary central breast cancer underwent locally extended excision and axillary lymph node dissection. Nipple-areola complex was excised concurrently. Patients received whole breast radiotherapy, radiotherapy, endocrinal therapy or Herceptin target therapy based on the pathological and immunohistochemical analysis of the results of conventional whole breast radiotherapy. Results The follow-up of 7 - 96 months （ median, 38 months） found no local recurrence or distant metastasis. The cosmetic effect of the conserved breast was satisfactory, with 53.5% （23/43） being rated excellent and 32.6% （14/43） good. Conclusions Breast conservation therapy of nipple-areola complex resection is feasible for early primary central breast cancer if surgical indications are strictly observed. The short-term results of the surgery are satisfactory, while long-term follow-up is still needed for the final outcome.
