应激有关因素与缓解期精神分裂症患者心理健康的相关性

摘　　要：目的：探讨应激有关因素与缓解期精神分裂症患者心理健康的相关性。方法：采用症状自评量表、社会支持量表、应对方式量表、负性自动思维问卷及艾森克人格量表对149例缓解期精神分裂症患者进行评定，分析应激有关因素对心理健康的影响。结果：分析显示，应激有关因素中负性自动思维、自责、回避、神经质、幻想、精神质和合理化与患者心理健康存在显著相关（r=0．181～0．772）；起主要作用的应激因素依次是负性自动思维、合理化应付方式、内外向、神经质及社会支持（beta分别=0．595，0．213，-0．196，0．250，-0．114）。通径分析显示，负性自动思维主要直接作用于患者的心理健康（通径系数为0．595），少部分间接通过社会支持（通径系数为0．018）作用于患者的心理健康；神经质个性既可直接、也可间接通过其他因素作用于患者的心理健康。结论：应激有关因素对缓解期精神分裂症患者的心理健康可能存在一定影响，其中负性自动思维及神经质个性起了较重要的作用。Objective：To explore the correlation between stress-related factors and psychiatric health in remitted schizophrenia patients. Method ： The symptom checklist-90 （SCL-90） , social support scale, coping style scale,automatic thoughts scale and Eysenck personality questionnaire were used to assess 149 schizophren- ics in remission in order to analyze the efficacies of stress-related factors on mental health in patients with schiz- ophrenia in remission. Results：The analysis showed that nagative automatic thought, self-abuse, avoidance, neuroticism illusion,psychoticism,rationalization were significantly associated with the psychiatric health status in patients （ r = 0. 181-0. 772）. The main stress factors that influence the psychiatric health in patients were negative automatic thoughts, rational coping style, introversion and extroversion, ueuroticism and social support （ beta = 0. 595,0.213, - 0.196,0. 250, - 0.114, respectively）. Path analysis showed most of negative automat- ic thoughts had direct impact on the psychiatric health in patients（ path coefficient = 0.595 ） while a part of neg- ative automatic thoughts had indirect impact on that by social support（ path coefficient = 0. 018） ; neuroticism was directly or indirectly affected on the psychiatric health. Conclusion：Stress-related factors perhaps influ- ence the psychiatric health in patients with schizophrenia in remission, in which the negative automatic thoughts and neuroticism play the most important roles.
