脑卒中后抑郁症干预治疗研究

摘　　要：目的探讨心理干预加药物抗抑郁治疗对脑卒中后抑郁症神经功能恢复及心理健康水平的影响。方法对304例确诊脑卒中患者进行抑郁自评量表(SDS)、汉密尔顿抑郁量表(HAMD)、心理健康调查表(PHI)以及中国脑卒中临床神经功能缺损程度量表(CSS)评分,将确诊为抑郁症的随机分为对照组及治疗组,两组均进行神经内科常规治疗,治疗组增加心理干预加药物抗抑郁治疗,治疗1个月后对两组SDS、HAMD、PHI及CSS的结果比较。结果治疗组HAMD、SDS、CSS各量表的总分分别为(36.260±9.547)、(13.260±7.303)、(6.930±4.166)均低于对照组的(51.500±6.557)、(17.660±9.565)、(9.500±6.659)(P=0.018,0.000,0.034),治疗组PHI的L、F、SOM、DEP、ANX、PSD、HYP、UNR、HMA分分别(3.420±1.295)、(5.090±1.674)、(10.420±2.239)、(9.630±1.773)、(4.560±1.750)、(5.090±2.328)、(5.090±2.328)、(5.190±2.538)、(5.140±2.660)低于对照组的(4.390±1.316)、(9.360±8.297)、(11.860±3.032)、(13.140±2.699)、(8.430±2.856)、(8.110±3.336)、(8.110±3.336)、(6.680±2.709)、(8.250±3.390)(P=0.001,0.001,0.013,0.000,0.000,0.000,0.000,0.009,0.000)。结论心理干预加药物抗抑郁治疗对脑卒中后抑郁症神经功能恢复及心理健康水平的提高有重要作用。[Objective] To study mental intervention and antidepressant drugs impact on the recovery of neurological function and mental health for post stroke depression. [Methods] 304 stroke patients were evaluated by SDS, HAMD, PHI and CSS. The diagnosis of depression was randomly divided into control group and treatment group. Both groups were treated for routine neurology. The treatment group was increased psychological intervention and antidepressant drugs. After Treatment for 1 months, the result was compared for the two SDS, HAMD, PHI and CSS. [Resultsl Treatment group HAMD, SDS, CSS total score of each scale （every was 36.260±9.547, 13.260± 7.303, 6.930±4.166） were lower than the control group （every was 51.500±6.557, 17.660±9.565, 9.500±6.659）（P = 0.018, 0.000, 0.034）. Treatment group of PHI L, F, SOM, DEP, ANX, PSD, HYP, UNR, HMA points （every was 3.420±1.295, 5.090±1.674, 10.420±2.239, 9,630±1.773, 4.560±1.750, 5.090±2.328, 5.090±2.328, 5.190±2.538, 5.140±2.660） were lower than the control group （every was 4.390±1.316, 9.360±8.297, 11.860±3.032, 13.140±2.699, 8.430±2.856, 8.110±3.336, 8.110±3.336, 6.680±2.709, 8.250±3.390） （P =0.001, 0.001, 0.013, 0.000, 0.000, 0.000, 0.000, 0.009, 0.000）. [ Conclusions] Psychologieal intervention and antidepressant medication treatment for post stroke depression neurological reeovery and improvement of mental health plays an important role.
