摘 要:目的探讨TNF-α基因多态性与非小细胞肺癌发生的关系。方法对60例非小细胞肺癌患者(肺癌组)及62例体检的健康者(对照组)空腹采集外周静脉血2 mL,采用PCR-RFLP方法分析2组TNF-α基因多态现象。结果肺癌组与对照组均未检出TNF-α-308 A/A及TNF-α-238 A/A纯合子。肺癌组TNF-α-308及TNF-α-238的基因型频率分布与对照组比较差异均有统计学意义(均P<0.05)。TNF-α-308的基因型频率分布:鳞癌组与腺癌组、Ⅰ+Ⅱ期组与对照组的G/G基因型与G/A基因型在分布频率上比较差异均无统计学意义(均P>0.05);Ⅲ+Ⅳ期组与对照组比较差异有统计学意义(均P<0.05)。TNF-α-238的基因型频率分布:鳞癌组、腺癌组G/G基因型与G/A基因型频率分布与对照组比较差异均有统计学意义(P<0.05)。Ⅰ+Ⅱ期组与对照组的基因型分布频率比较差异无统计学意义(P>0.05);Ⅲ+Ⅳ期组与对照组的基因型分布频率上比较差异有统计学意义(P<0.05)。结论 TNF-α-308 G/A及TNF-α-238 G/A基因型与非小细胞肺癌的发生有关。其中-308 A等位基因对非小细胞肺癌的发生发展有推动作用,-238 A等位基因则有保护性作用。Objective To investigate the association of TNF-ot polymorphism with non-small cell lung cancer susceptibility. Methods Fasting venous blood samples (2 mL) were collected from 60 patients with lung cancer (lung cancer group) and 62 healthy subjects (control group). TNF-α polymorphism was analyzed using PCR-RFLP method in both groups. Results Neither TNF-ot-308 A/ A nor TNF-α-238 A/A homozygote was detected in both groups. There were significant differences in TNF-a-308 and TNF-α-238 genotype distributions between lung cancer group and control group (P〈 0.05). However, no significant differences in TNF-α-308 G/G and TNF-or-308 G/A genotype distributions were found between squamous carcinoma group and adenocarcinoma group, as well as between stage I +II group and control group (P〉O.05).TNF-oL-308 G/G and TNF-ot-308 G/A genotype distributions in stage 11I +IV group were obviously different from those in control group (P〈0.05). There were significant differences in TNF-ot-238 G/G and TNF-ot-238 G/A genotype distributions between squamous carcinoma group or adenocarcinoma group and control group,as well as between stage Ill+IV group and control group(P〈0.05).Condusion Both TNF-α-308 G/A and TNF-α-238 G/A polymorp- hisms are correlated with the occurrence of non-small cell lung cancer. The -308 A allele can promote the development of lung cancer but -238 A allele can play protective role in lung cancer.
