摘 要:目的探讨类胰岛素样生长因子1(IGF-1)与明胶微球(GMs)复合物对兔下颌骨缺损新骨形成的影响。方法日本成年大耳白兔共27只,随机分成3组,各组均在一侧下颌骨体部制备一13 mm×6 mm大小的单侧皮质骨缺损模型,实验组于缺损区植入IGF-1/GMs复合物,对照组植入GMs,空白对照组不作特殊处理。术后4、8、12周每组各处死3只获取标本行大体解剖观察、X线、骨密度测定、苏木素-伊红(HE)染色及骨小梁百分比测定。结果 HE染色显示实验组骨缺损早期(4周)见少量不成熟骨小梁、胶原成分及大量新生血管;8周时,各组均可见成熟的骨小梁,实验组骨小梁粗大,板层状排列,对照组骨小梁明显少于实验组;12周时实验组骨质基本接近成熟,与正常骨质无明显分界。实验组骨密度及新生骨面积百分比在各时间点明显高于对照组和空白组,差异有统计学意义。结论 IGF-1/GMs复合物能促进颌骨缺损早期修复,具有良好的生物相容性,有临床应用价值。Objective To investigate the effects of insulin-like growth factor 1 (IGF-1)and gelatin mierospheres (GMs) complex on new bone formation in rabbits with mandibles defect. Methods Twenty-seven adult Japanese white rabbits were, following construction of a 13 mm× 6 mm unilateral cortical bone defect at the mandible, random- ly assigned to be treated with implantation of IGF-1/GMs (experimental group), empty GMs (control group) or nil mate- rial (blank control group), respectively. Three rabbits were sacrificed at 4, 8 and 12 weeks in each group. This en- tailed gross anatomical observations, X-ray, bone densitometry, HE staining and measurement of the occupancy of tra- becular bone. Results Pathological staining showed that, minor immature trabeeular bone, collagen and massive an- giogenesis were noted at the early stage in experimental group (4 weeks). At week 8, each group yielded marked os- teogenesis, as evidenced by a mature trabecular bone with lamellar arrangements in the experimental group yet signifi- cantly less trabecular bone formation in the control group. At week 12, the experimental group had a virtually mature bone formation with an unmarked boundary with normal tissues. The experimental group also yielded a significantly higher bone mineral density and the occupancy of new bone area at all time points compared with control group and blank control group (both P〈0.05). Conclusion The IGF-1/gelatin microspheres compound promotes early repair of mandible defects and has a preferable biocompatibility, suggesting that it is a promising material for future clinical ap- plications.
