影响白内障患者选择人工晶体因素的分析

摘　　要：目的植入合适的人工晶体对白内障患者的术后视力恢复十分重要,文中通过分析影响白内障患者选择不同人工晶体的因素,指导患者及医师合理选择人工晶体。方法回顾性分析我院1510例白内障患者的一般情况及术眼情况,并对可能影响患者选择人工晶体的因素进行多因素Logistic回归分析。结果单因素分析显示职业、眼部病变、术前视力、手术医师在影响白内障患者选择人工晶体方面差异具有统计学意义(P<0.05),性别、年龄、健康状况差异无统计学意义(P>0.05)。多因素Logistic回归分析显示手术医师、职业、术前视力及眼部病变是影响患者选择不同类型人工晶体的重要因素。结论脑力劳动、术前视力<0.1及无眼部病变患者倾向选择多焦点及非球面人工晶体,不同医师选择人工晶体倾向不同,术前应综合考虑以上因素,帮助患者合理地选择人工晶体。Objective Lens implant plays an important role in vision recovery for the patients with catarct. The aim of this study is to analyze the impact factors of selection of intraocular lens （ IOL ） on cataract patients, to understand the needs of patients and their constraints, and to guide both the patient and doctor a reasonable choice of IOL. Methods Based on retrospective analyzes 1510 cases of cataract patients in general and ocular conditions in Jinling hospital, multi-factors Logistic regression analysis was used to find the impact factor of selection of IOL. Results The single factor Logistic regression analysis showed the significant differences in job, eye disease, preoperative visual acuity and surgeon （ P 〈 0.05 ） , but there were no differences in the sex, age and health condition of patients （ P 〉 0.05 ）. By logistic regression analysis, the impact factors of selection of IOL were surgeon, preoperative visual acuity, job and eye disease. Conclusion Patients with mental work, preoperative visual less than 0.1 and non-eye disease are likely to choose mutifocal intraocular lens and aspheric intraocular lens and surgeon also affect the patient＇s selection. Those factors should be taken into consideration before surgery to help patients select the IOL advisably.
