摘 要:目的分析甲状腺良恶性肿瘤多层螺旋CT(MSCT)的特征,探讨MSCT在其鉴别诊断中的价值。方法选择2006年10月至2012年10月诊治的甲状腺结节患者80例,其中甲状腺癌32例,甲状腺良性肿瘤48例。所有患者均于入院3d内行MSCT检查。观察不同性质甲状腺肿瘤的MSCT特征,包括部位、数量、形态、边缘、密度、与邻近组织关系及强化特征。结果 MSCT对甲状腺良恶性肿瘤诊断准确率为84.4%,特异性为90.6%。MSCT显示良性甲状腺肿瘤平均直径约(2.56±0.97)cm,恶性甲状腺肿瘤平均直径约(2.46±0.89)cm,两者差异无统计学意义(t=0.342,P>0.05)。良性甲状腺肿瘤边缘清晰、多发、增强后边缘清晰及有包膜的比例(91.7%、60.4%、83.3%和85.4%)均显著高于恶性甲状腺肿瘤(31.3%、15.6%、28.1%和34.4%),而恶性甲状腺肿瘤内细颗粒钙化的比例(28.1%)显著高于良性甲状腺肿瘤(4.2%),以上各指标比较差异均有统计学意义(均P<0.05)。结论 MSCT作为无创的检测手段在对甲状腺良恶性肿瘤的鉴别中具有较高的应用价值,可从肿瘤的边缘、形态、包膜、钙化及与邻近组织关系等方面进行鉴别,为临床甲状腺肿瘤的鉴别诊断提供依据。Objective To analyze the Multislice CT( MSCT) characteristics of benign and malignant thyroid lesion,explore the value of MSCT in the differential diagnosis. Methods 80 patients with thyroid nodules in our hospital general surgical department from October 2010 to October 2012 were chosen for the study,there were 32 cases of thyroid cancer,48 cases of benign thyroid. All patients were underwent MSCT examination within 3d admitted to hospital. the different nature of thyroid tumors MSCT characteristics,including location,number,shape,edge,density,and strengthening relations with neighboring tissue characteristics were compared. Results MSCT benign and malignant thyroid diagnostic accuracy rate was 84. 4%,specificity rate was 90. 6%. MSCT benign thyroid tumors show an average diameter of about( 2. 56 ±0. 97) cm,malignant thyroid tumors average diameter were( 2. 46 ±0. 89) cm,there were no significant difference between them( t = 0. 342,P〈0. 05). Benign thyroid tumors sharp edges,multiple, sharp edges and enhanced enveloped proportion( 91. 7%,60. 4%,83. 3%,85. 4%) were significantly higher than malignant thyroid tumors( 31. 3%,15. 6%,28. 1%,34. 4%),and malignant thyroid tumor calcification proportion of fine particles( 28. 1%) was significantly higher than benign thyroid tumors( 4. 2%),the differences were all statistically significant( P〈0. 05). Conclusions MSCT has a higher value in the identification of benign and malignant thyroid,from the edge of the tumor,shape,capsule,calcification and relations with neighboring organizations and other aspects of identification.
